血栓弹力图预测脑梗死静脉溶栓后早期神经功能恶化的研究进展 被引量:1

摘要 静脉溶栓后发生早期神经功能恶化(END)严重影响患者神经功能恢复及临床预后,但目前缺乏静脉溶栓后END病情评估方法及预测指标。血栓弹力图参数R值减小及MA值升高提示脑梗死静脉溶栓后血液存在高凝状态,但国内外对血栓弹力图预测脑梗死静脉溶栓后END价值的研究结果尚不完全一致。通过血栓弹力图检测凝血功能可能有助于预测静脉溶栓相关出血风险,但不能预判静脉溶栓治疗的临床反应及提供治疗指导。血栓弹力图能否成为静脉溶栓后END的预测方法仍需要大规模临床循证学依据证明。本文就血栓弹力图在预测脑梗死静脉溶栓后END的研究进展进行综述。 Early neurological deterioration(END)after intravenous thrombolysis seriously affects the neurological recovery and clinical prognosis of patients.However,there are currently no assessment methods and predictive indicators for END after intravenous thrombolysis.Decreased R value and increased MA value of thromboelastography parameters suggest that the blood is hypercoagulable after intravenous thrombolysis in cerebral infarction.While,the results of domestic and foreign studies on the value of thromboelastography in predicting END after intravenous thrombolysis in cerebral infarction are not completely consistent.Detection of coagulation function by thromboelastography may help to predict the bleeding risk associated with intravenous thrombolysis,but it cannot predict the clinical response of intravenous thrombolysis and provide treatment guidance.Whether thromboelastography can be used as a predictor of END after intravenous thrombolysis still needs large-scale clinical evidence-based evidence.This article reviews the research progress of thromboelastography in predicting END after intravenous thrombolysis in cerebral infarction.
