摘要
目的:对比CT与磁共振灌注成像对缺血性脑血管病患者的诊断价值。方法:2018年3月-2020年12月100例可疑缺血性脑血管病患者均给予CT灌注成像、磁共振灌注成像及血管造影检查,比较CT灌注成像、磁共振灌注成像的灌注参数及诊断效能。结果:CTPI显示CI患侧CBF、CBV较健侧均显著降低,TTP、MTT较健侧均显著提高,TIA患侧TTP、MTT较健侧均显著提高,差异均有统计学意义(P<0.05);其余参数对比差异均无统计学意义(P>0.05)。PWI显示CI、TIA患侧CBF较健侧均显著降低(P<0.05),其余参数对比差异均无统计学意义(P>0.05)。DSA检查阳性病灶共计321个,其中CTPI阳性病灶检出率为91.28%,PWI阳性病灶检出率为72.59%,以DSA诊断结果作为金标准,计算CTPI诊断缺血性脑血管病阳性病灶的灵敏度为90.65%、特异度为87.50%、符合率为90.50%;PWI诊断缺血性脑血管病阳性病灶的灵敏度为70.72%、特异度为62.50%、符合率为70.33%。CTPI检查灵敏度和符合率均显著高于PWI检查(P<0.05),但两者特异度对比差异无统计学意义(P>0.05)。结论:CT和磁共振灌注成像对于缺血性脑血管病诊断价值均较高,但是CTPI诊断灵敏度和符合率更高,且操作简单,患者的配合度高,值得推广。
Objective:To compare and analyze the diagnostic value of CT and magnetic resonance perfusion imaging in patients with ischemic cerebrovascular disease.Method:From March 2018 to December 2020,100 patients with suspected ischemic cerebrovascular disease were selected for CT perfusion imaging(CTPI),magnetic resonance perfusion imaging(PWI)and angiography(DSA),the perfusion parameters and diagnostic performance of CT and magnetic resonance perfusion imaging were compared.Result:CTPI showed that CBF and CBV in the affected side of CI were significantly lower than those in the healthy side,TTP and MTT were significantly higher than those in healthy side,and TTP and MTT in the affected side of TIA were significantly higher than those in the healthy side,the differences were statistically significant(P<0.05);there were no significant differences in other parameters(P>0.05).PWI showed that CBF in the affected side of CI and TIA were significantly lower than that in the healthy side(P<0.05),there were no significant differences in other parameters(P>0.05).A total of 321 positive lesions were detected by DSA,the detection rate of CTPI positive lesions was 91.28%,the detection rate of PWI positive lesions was 72.59%,using DSA diagnosis result as the gold standard,the sensitivity of CTPI for diagnosing ischemic cerebrovascular disease positive lesions was 90.65%,the specificity was 87.50%,and the coincidence rate was 90.50%;the sensitivity of PWI for diagnosing ischemic cerebrovascular disease positive lesions was 70.72%,specificity was 62.50%,coincidence rate was 70.33%.CTPI examination sensitivity and coincidence rate were significantly higher than those of PWI examination(P<0.05),the specificity between CTPI examination and PWI examination was not statistically significant(P>0.05).Conclusion:CT and magnetic resonance perfusion imaging are effective for ischemic brain Vascular diseases are of high diagnostic value,but CTPI has higher diagnostic sensitivity and coincidence rate,simple operation,and high patient cooperation,w
