螺旋CT血管造影检查在心肌桥——壁冠状动脉血管形态学诊断中的应用 被引量:2

摘要 目的探讨螺旋CT血管造影(CTA)检查在心肌桥—壁冠状动脉血管形态学诊断中的应用价值。方法选取2017年1月至2020年7月医院收治的87例心肌桥—壁冠状动脉确诊患者,均行CTA检查,以冠状动脉造影(CAG)检查结果为金标准,分析CTA检查的诊断效能。结果 CTA检查深在型和浅在型心肌桥—壁冠状动脉患者的心肌桥长度分别为(10.05±3.07)mm和(7.62±2.15)mm,均高于CAG检查的(8.06±3.21)mm和(4.56±2.19)mm,差异有统计学意义(t=4.179、9.300,P<0.05)。CTA检查深在型和浅在型心肌桥—壁冠状动脉患者的壁冠状动脉狭窄程度分别为(51.43±12.69)%和(29.82±11.42)%,与CAG检查的(52.54±12.43)%和(30.42±12.55)%比较,差异无统计学意义(t=0.583、0.330,P>0.05)。以CAG检查结果为金标准,CTA诊断深在型和浅在型心肌桥—壁冠状动脉的准确度、灵敏度、特异度分别为82.76%、78.95%、85.71%和82.76%、85.71%、78.95%。结论在心肌桥—壁冠状动脉的形态学诊断中应用CTA检查可较为准确地判断心肌桥长度、壁冠状动脉狭窄程度,还可准确鉴别疾病的深在型和浅在型,为临床早期治疗提供依据。 Objective The application of spiral CT angiography(CTA) in the morphological diagnosis of myocardial bridge-mural coronary artery was explored. Methods From January 2017 to July 2020, 87 patients with myocardial bridge mural coronary artery admitted to the hospital were selected as the subjects. All patients underwent CTA examination. The results of coronary angiography(CAG) were taken as the gold standard to analyze the diagnosing efficacy of CTA. Results The lengths of myocardial bridge were(10.05±3.07) mm and(7.62±2.15) mm in patients with deep and superficial myocardial bridge-mural coronary artery by CTA, which were both longer than(8.06±3.21) mm and(4.56±2.19)mm by CAG, with statistically significant differences(t=4.179, 9.300, P<0.05). The degrees of stenosis of myocardial bridge in patients with deep and superficial myocardial bridge-mural coronary artery by CTA were(51.43±12.69)% and(29.82±11.42)%,and they were(52.54±12.43)% and(30.42±12.55)% by CAG, and the differences were not statistically significant(t=0.583,0.330, P>0.05). Taking the results of CAG as the gold standard, the accuracy, sensitivity and specificity of CTA in the diagnosis of deep and superficial myocardial bridge-mural coronary artery were 82.76%, 78.95%, 85.71%, and 82.76%,85.71%,78.95%,respectively. Conclusion The application of CTA in the morphological diagnosis of myocardial bridge-mural coronary artery can accurately judge the length of myocardial bridge and the degree of stenosis of mural coronary artery, and can accurately distinguish the superficial type and deep type of this disease, thus providing useful guidance for the early treatment.
