摘要
目的探讨黄葵胶囊联合西医治疗糖尿病肾病(Diabetic Nephropathy,DN)Ⅲ、Ⅳ期的临床效果。方法选取2016年8月—2020年7月期间于常州市武进人民医院中医科门诊及住院治疗的128例Ⅲ、Ⅳ期DN患者,按随机数字表法将其随机分成观察组和对照组,每组各64例。两组患者均给予常规西药治疗,观察组在此基础上口服黄葵胶囊治疗,所有患者均治疗12周。观察比较两组患者临床疗效,治疗前后氧化应激指标[丙二醛(Malondialdehyde,MDA)、超氧化物歧化酶(Superoxide dismutase,SOD)、8-羟化脱氧鸟苷(8-hydroxy-2 deoxyguanosine,8-OHdG)]肾组织匀浆浓度及肾功能指标[血清尿素氮(BUN)、肌酐(Cr)水平和24 h尿蛋白定量(24 h UP)]和血清血管内皮细胞功能指标[血管内皮生长因子(Vascular endothelial growth factor,VEGF)、内皮素-1(Endothelin-1,ET-1)、一氧化氮(Nitric Oxide,NO)]水平变化,记录治疗期间不良反应情况。结果治疗后观察组总有效率为90.63%(58/64),对照组为71.88%(46/64),两组比较,差异有统计学意义(P<0.05)。两组患者治疗后肾组织匀浆MDA浓度均较治疗前明显降低(P<0.05),SOD水平均较治疗前明显升高(P<0.05);观察组治疗后肾组织匀浆8-OHdG含量较治疗前明显下降(P<0.05),但对照组无明显变化(P>0.05)。且治疗后观察组肾组织匀浆MDA、8-OHdG水平均较对照组明显降低(P<0.05),SOD水平较对照组明显升高(P<0.05)。两组患者治疗后血清BUN、24 h UP含量均较治疗前明显减少(P<0.05),观察组治疗后血清Cr水平较治疗前明显降低(P<0.05),而对照组无明显改变(P>0.05)。且治疗后观察组血清BUN、Cr、24 h UP含量均较对照组明显降低(P<0.05)。两组患者治疗后血清VEGF、ET-1水平均较治疗前明显降低(P<0.05),而NO含量则均明显升高(P<0.05);且观察组改善更明显(P<0.05)。观察组不良反应发生率6.25%(4/64)与对照组4.69%(3/64)比较,差异无统计学意义(χ^(2)=0.000,P=1.000,P>0.05)。结论
Objective To investigate the clinical efficacy of Huangkui capsule combined with western medicine on patients with diabetic nephropathy(DN)in stageⅢ,Ⅳ.Methods A total of 128 patients with stage III and IV DN who were treated in the Department of Traditional Chinese Medicine of Wujin People′s Hospital in Changzhou from August 2016 to July 2020,were divided into observation group(64)and control group(64)according to the random number table.Both groups were treated with conventional western medicine,in addition to which the observation group received oral administration of Huangkui capsule.All patients were treated for 12 weeks.The clinical efficacy,changes of oxidative stressindicators[malonaldehyde(MDA),superoxide dismutase(SOD),8-hydroxy-2 deoxyguanosine(8-OHDG)]kidney homogenate concentration,renal function indexes[serum urea nitrogen(BUN),creatinine(Cr)and 24 h urine protein quantitation(24 h UP)],serum endothelial function parameters[ascular endothelial growth factor(VEGF),endothelin-1(ET-1),nitric oxide(NO)]levels before and after treatment,and side effect were compared between the two groups.Results After treatment,the total effective rate in the observation group was 90.63%(58/64),which was 71.88%(46/64)in the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the MDA concentration in renal tissue homogenate of the two groups of patients was significantly lower than that before treatment(P<0.05),and the SOD level was significantly higher than that before treatment(P<0.05);the 8-OHdG content of renal tissue in the observation group after treatment It was significantly lower than before treatment(P<0.05),but there was no significant change in the control group(P>0.05).After treatment,the levels of MDA and 8-OHdG in the renal homogenate of the observation group were significantly lower than those of the control group(P<0.05),and the SOD level was significantly higher than that of the control group(P<0.05).Serum BUN and 24 h UP levels of the two group
