摘要
目的通过Meta分析评价中西医结合治疗心肾综合征的有效性。方法检索中国知识基础设施工程(CNKI)、万方数据知识服务平台(Wanfang Data)、维普中文期刊服务平台(VIP)、PubMed和Webofscience数据库,纳入中西医结合治疗心肾综合征的随机对照临床研究,时间从建库截至2021年5月,经筛选后最终纳入28个研究,采用Cochrane协作网的偏倚风险评估工具进行文献质量评价,RevMan5.3软件进行Meta分析。结果与常规西医治疗方案相比,中西医结合治疗心肾综合征在提高总有效率、提高LVEF、减小LVEDD、降低NT-BNP或BNP、降低Scr、BUN、CysC和提高e GFR及改善炎症反应方面均优于单纯西药治疗。结论基于目前证据,充分应用辨证论治理论,选用合适的中药方剂才能更好地提高心肾综合征的治疗效果,但仍需要高质量证据予以验证。
Objective:To evaluate the effectiveness of integrated traditional Chinese and western medicine in the treatment of cardiorenal syndrome through meta-analysis.Methods:Databases Chinese Knowledge Infrastructure Engineering(CNKI),WanFang Data Knowledge Service Platform(WanFang Data),VIP Chinese Journal Service Platform(VIP),PubMed and Web of Science were searched from inception to May 2021 for randomized controlled clinical studies of cardiorenal syndrome treated by integrated Chinese and Western medicine.A total of 28 studies were finally included after screening.The bias risk assessment tool of the Cochrane Collaboration was used for literature quality evaluation,and the RevMan 5.3 software was used for Meta analysis.Results:The results of the meta-analysis showed that compared with conventional Western medicine treatment,the treatment of cardiorenal syndrome with integrated Chinese and Western medicine had better effects in improving the total effective rate,increasing LVEF,decreasing LVEDD,NT-BNP or BNP,Scr,BUN,CysC,and improving eGFR and inflammatory response than western medicine alone.Conclusion:Based on the current evidence,adequate application of the theory of syndrome differentiation and the selection of appropriate Chinese medicines can better improve the treatment efficacy of cardiorenal syndrome,but high-quality evidence is still needed for verification.
