摘要
目的探讨无创正压机械通气在急性左心衰竭患者急救中的临床效果。方法74例急性左心衰竭患者,使用随机数字表法分为对照组和观察组,每组37例。急救时,对照组采用常规综合治疗措施,观察组在对照组基础上联合无创正压机械通气治疗。对比两组临床疗效以及治疗前后血气指标[pH值、动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]变化情况。结果观察组治疗总有效率97.30%(36/37)明显高于对照组的81.08%(30/37),差异有统计学意义(P<0.05)。治疗后,两组患者pH值、PaO_(2)高于本组治疗前,PaCO_(2)低于本组治疗前,且观察组患者p H值(7.38±0.13)、PaO_(2)(91.75±8.61)mm Hg(1 mm Hg=0.133 kPa)高于对照组的(7.30±0.12)、(74.22±7.13)mm Hg,PaCO_(2)(41.74±2.12)mm Hg低于对照组的(51.32±4.13)mm Hg,差异具有统计学意义(P<0.05)。结论对急性左心衰竭患者急救时在常规药物治疗的同时采用无创正压机械通气治疗可以显著提升治疗效果,改善血气指标,值得推荐使用。
Objective To discuss the clinical effect of non-invasive positive-pressure mechanical ventilation on patients with acute left heart failure.Methods A total of 74 patients with acute left heart failure were divided into control group and observation group according to random numerical table,with 37 cases in each group.During the emergency treatment,the control group was treated with conventional comprehensive treatment measures,and the observation group was treated with non-invasive positive-pressure mechanical ventilation on the basis of the control group.The clinical efficacy and changes of blood gas indexes[pH,arterial partial pressure of oxygen(PaO_(2)),and arterial partial pressure of carbon dioxide(PaCO_(2))]before and after treatment were compared between the two groups.Results The total effective rate 97.30%(36/37)in the observation group was significantly higher than 81.08%(30/37)in the control group,and the difference was statistically significant(P<0.05).After treatment,the pH and PaO_(2) of the two groups were higher than that of this group before treatment,and PaCO_(2) was lower than that of this group before treatment;the pH value(7.38±0.13),PaO_(2)(91.75±8.61)mm Hg(1 mm Hg=0.133 kPa)of the observation group were higher than(7.30±0.12)and(74.22±7.13)mm Hg of the control group,and the PaCO_(2)(41.74±2.12)mm Hg was lower than(51.32±4.13)mm Hg of the control group.All differences were statistically significant(P<0.05).Conclusion For patients with acute left heart failure,noninvasive positive-pressure mechanical ventilation on the basis of conventional drug therapy can significantly improve the therapeutic effect and blood gas index,which is worthy of recommendation.
