摘要
目的探讨经鼻高流量氧疗对急性左心衰患者的临床疗效。方法选取2017年10月~2019年10月我院收治的46例急性左心衰竭患者为研究对象,依据给氧方式不同分为经鼻高流量氧疗组(high-flow nasal cannula oxygen therapy,HFNC)和无创正压通气组(non-invasive positive pressure ventilation,NIPPV),每组23例。两组患者均接受急性左心衰竭标准治疗,观察比较两组治疗前后呼吸循环指标、心功能指标、不良并发症(胃肠障碍、痰液黏稠)、舒适度及气管插管率。结果治疗后,两组患者呼吸循环指标及心功能指标均较治疗前显著改善,差异均有统计学意义(P<0.05);而且两组治疗后的呼吸循环指标、心功能指标及气管插管率比较,差异均无统计学意义(P>0.05)。HFNC组的治疗舒适度高于NIPPV组,不良并发症(胃肠胀气及痰液黏稠)发生率低于NIPPV组,差异均有统计学意义(P<0.05)。结论HFNC能有效改善急性左心衰竭患者的氧合及心功能,患者舒适性更高,不良并发症更少,值得临床应用。
Objective To explore the cilinical efficacy of high-flow nasal cannula oxygen therapy(HFNC)in patients with acute left heart failure(ALHF).Methods From October 2017 to october 2019,46 patients with acute left heart failure were randomly divided into high flow oxygen inhalation group(group HFNC)and non-invasive positive pressure ventilation group(group NIPPV)as the different ways of given oxygen,and with 23 cases in each group.Patients were given standard treatment of acute left heart failure.Respiratory circulation indexs,adverse complications(gastroin-testinal distention,stiffer sputum),comfortableness and intubation of two groups were observed after treatment.Results The PaO 2,PO 2/FiO 2,LVEF,SV of patients in two groups after treatment were significant higher than before;the HR,RR,MAP,PCO 2 and NT-proBNP of patients in two groups were significant lower than before(P<0.05).There was no significant difference in the HR,RR,MAP,PO 2,PCO 2,PO 2/FIO 2,LVEF,SV,NT-proBNP and the rate of intubation after treatment between the two groups(P>0.05).HFNC group has better comfortably and lower adverse complicatons than NIPPV group(P<0.05).Conclusion HFNC can improve oxygenation and cardiac functions in patients with acute left heart failure,is more comfortable and less adverse complicationis than NIV,worthy of being promoted in clinical practice.
