摘要
目的探讨基于磁锚定技术原理在膀胱黏膜下注射磁性水凝胶用于非肌层浸润性膀胱癌切除的可行性。方法以4个新鲜离体猪膀胱为模型,将自行配制的海藻酸钠-四氧化三铁微粒混悬液和100 g/L葡萄糖酸钙溶液按体积比1∶1注射到假定的膀胱黏膜肿瘤下方,然后在体外锚定磁体的吸引牵拉下充分显露膀胱黏膜下层,从而完成对假定病变黏膜的完整切除。结果在4个新鲜离体猪膀胱上,可顺利将海藻酸钠-四氧化三铁混悬液和钙剂注射到膀胱病变黏膜下,两者相遇后迅速形成凝胶状。在锚定磁体对磁性水凝胶的吸引牵拉下,病变黏膜下层充分显露,极大地方便了内镜下病灶的切除。结论基于磁锚定技术的磁性水凝胶膀胱黏膜下注射可充分显露术野,极大改善内镜下操作体验,实现病灶的整块切除。
Objective To investigate the feasibility of magnetic hydrogel submucosal injection for non-muscle-invasive bladder cancer(NMIBC)resection based on magnetic anchor technique(MAT).Methods A total of 4 fresh pig bladders were used as models,and a self-formulated alginate-ferric oxide particulate suspension and 100 g/L calcium gluconate solution were injected at a 1∶1 volume ratio into the hypothetic bladder mucosal tumor.Then the bladder submucosa was fully exposed under the attraction and pull of anchor magnet on the outside,so as to complete the resection of the presumed diseased mucosa.Results The sodium alginate ferric oxide solution and calcium agent were smoothly injected into the submucosa of bladder lesions,and gelatin formed quickly.Under the pull of magnetic hydrogel attracted by the anchor magnet,the lesion submucosa was fully exposed,which greatly facilitated the endoscopic resection of the lesion.Conclusion Submucosal injection of magnetic hydrogel into the bladder based on MAT can fully expose the operative field,greatly improve the endoscopic operation experience,and realize whole lesion resection.
