摘要
目的探讨311 nm窄谱中波紫外线比率加量与固定加量对银屑病患者血MMP-13和IL-17水平的影响。方法选取2018年10月至2020年10月在本院确诊的86例银屑病患者,采用随机数表法将患者分为观察组43例和对照组43例。对照组患者采用311 nm窄谱中波紫外线固定加量治疗,观察组采用311 nm窄谱中波紫外线比率加量治疗,均持续治疗4周。分别于治疗前后,采用银屑病面积和严重程度指数(PASI)对患者进行病情评分并评估疗效,采用酶联免疫吸附测定(ELISA)法测定外周血中MMP-13和IL-17的表达水平。记录两组治疗期间的不良反应发生情况。结果观察组治疗的总有效率为97.67%,明显高于对照组的83.72%(χ^(2)=4.962,P=0.026)。治疗后,两组患者外周血中MMP-13和IL-17表达水平均明显低于治疗前,且观察组MMP-13和IL-17表达水平低于对照组(P<0.05)。两组患者总不良反应发生率差异无统计学意义(χ^(2)=0.054,P=0.816)。结论311 nm窄谱中波紫外线比率加量与固定加量均可以明显降低MMP-13和IL-17表达水平,提高临床疗效,且效果优于固定加量法。
Objective To investigate the effect of 311 nm narrowband UVB ratio increase and fixed increase on blood MMP-13 and IL-17 levels in patients with psoriasis.Methods A total of 86 patients with psoriasis diagnosed in our hospital from October 2018 to October 2020 were selected,and the patients were divided into an observation group(n=43)and a control group(n=43)using a random number table method.The patients in the control group were treated with 311 nm narrow-band ultraviolet rays with a fixed dose,and the observation group was treated with 311 nm narrow-band ultraviolet rays with a ratio increase,and the treatment was continued for 4 weeks.Before and after treatment,the patients were scored by the Psoriasis Area and Severity Index(PASI)and the efficacy was evaluated,and the expression of MMP-13 and IL-17 in the peripheral blood was detected by the enzyme-linked immunosorbent assay(ELISA)method.level.The occurrence of adverse reactions during the treatment of the two groups was recorded.Results The total effective rate of treatment in the observation group was 97.67%,which was significantly higher than 83.72%in the control group(χ^(2)=4.962,P=0.026).After treatment,the expression levels of MMP-13 and IL-17 in the peripheral blood of the two groups of patients were significantly lower than before treatment,and the expression levels of MMP-13 and IL-17 in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of total adverse reactions between the two groups of patients(χ^(2)=0.054,P=0.816).Conclusion Both the ratio increase of 311 nm narrow-band ultraviolet light and the fixed dosage can significantly reduce the expression level of MMP-13 and IL-17,improve the clinical efficacy,and the effect is better than the fixed dosage method.
