非空腹血脂对冠心病诊断的预测价值

PREDICTIVE VALUE OF NON-FASTING BLOOD LIPID IN THE DIAGNOSIS OF CORONARY HEART DISEASE

摘要 目的预测非空腹血脂对冠心病诊断的价值。方法收集2018年3月—2020年3月该院心内科行冠脉造影检查的患者共280例的临床资料,所有患者常规饮食下,非空腹状态下采血4 ml,采用全自动生化分析仪测定血脂系列。应用Logistic回归评价增加冠心病发病风险的因素。应用ROC曲线分析和评价非空腹血脂对冠心病诊断的价值。结果根据冠脉造影结果分为冠心病组患者225例,其中男145例,女80例,平均年龄(61.48±10.40)岁,非冠心病组患者55例,其中男28例,女27例,平均年龄(63.76±10.58)岁。冠心病组非空腹状态下TG水平、非空腹LDL-C水平高于非冠心病组(1.97±1.00)mmol/L vs.(1.59±0.86)mmol/L、(3.22±1.01)mmol/L vs.(2.82±0.97)mmol/L(P<0.05)。Logistic回归分析结果显示,非空腹TG、非空腹LDL-C是冠心病的危险因素,OR值分别为1.559(95%CI:1.010~2.407)、1.548(95%CI:1.107~2.165)。非空腹TG、非空腹LDL-C诊断冠心病的ROC曲线下面积分别为0.627(95%CI:0.534~0.719)、0.608(95%CI:0.525~0.691)(P<0.001)。结论非空腹高TG、非空腹高LDL-C是冠心病的危险因素,两者对冠心病诊断均有一定的预测价值。 Objective To predict the diagnostic value of non-fasting blood lipids in people with coronary heart disease(CHD).Methods Collected the clinical data of cardiology inpatients during the period of March 2018 to March 2020.All patients were taken 4 ml blood under the normal diet and non-fasting state, and the blood lipids were measured by professionals.The factors of increased risk of CHD were used to analyze by Logistic regression.Logistic regression was used to evaluate the factors of increased risk of coronary heart disease.The predictive value of non-fasting blood lipids was analyzed and evaluated by using ROC curve.Results There were two hundred and twenty-five patients in the CHD group, including one hundred and forty-dive males and eighty females, with an average age of(61.48±10.40) years old.There were 55 patients in the non-CHD group, including 28 males and 27 females, with an average age of(63.76±10.58) years old.The levels of TG and LDL-C in a non-fasting state in the CHD group were higher than those in the non-CHD group(1.97±1.00) mmol/L vs.(1.59±0.86) mmol/L,(3.22±1.01) mmol/L vs.(2.82±0.97) mmol/L(P<0.05).Through Logistic regression analysis, we get that non-fasting TG and non-fasting LDL-C are risk factors for CHD and the OR values are 1.559(95%CI:1.010~2.407) and 1.548(95%CI:1.107~2.165).The area under the ROC curve of non-fasting TG and non-fasting LDL-C for the diagnosis of CHD were 0.627(95%CI:0.534~0.719)and 0.608(95%CI:0.525~0.691)(P<0.001).Conclusion Non-fasting TG and non-fasting LDL-C are risk factors for CHD and have certain value in predicting the diagnosis of CHD.
