益生菌对2型糖尿病患者恒定自然杀伤T细胞、血糖指标、炎性因子及肠道菌群的影响 被引量:5

摘要 目的探讨益生菌对2型糖尿病(T2DM)患者恒定自然杀伤T细胞、血糖指标、炎性因子及肠道菌群的影响。方法选取青岛滨海学院附属医院和浙江省立同德医院2018年3月至2020年12月收治的T2DM患者102例,随机分为观察组和对照组,各51例。两组患者均予常规治疗,并口服盐酸二甲双胍缓释片、阿卡波糖片,观察组患者加用益生菌干预。两组均治疗8周。结果观察组临床总有效率为94.12%,显著高于对照组的80.39%(P<0.05);观察组患者的恒定自然杀伤T(iNKT)细胞及白细胞介素4(IL-4)^(+)iNKT比例均显著高于对照组,双歧杆菌及乳杆菌数量显著多于对照组,γ干扰素(IFN-γ)^(+)iNKT比例、空腹血糖、糖化血红蛋白、餐后2 h血糖,超敏C反应蛋白、白细胞介素6及脂多糖水平均显著低于对照组,肠球菌数量及球杆菌数量均显著少于对照组(P<0.05);观察组与对照组不良反应发生率相当(11.76%比17.65%,P>0.05)。结论益生菌干预可改善T2DM患者的iNKT细胞水平、降低血糖和炎性因子水平,并改善肠道菌群失调状态。 Objective To investigate the effect of probiotics on invariant natural killer T(iNKT)cells,glycemic indexes,inflammatory factors and intestinal flora in patients with type 2 diabetes mellitus(T2DM).Methods A total of 102 patients with T2DM admitted to the Affiliated Hospital of Qingdao Binhai University and the Tongde Hospital of Zhejiang Province from March 2018 to December 2020 were selected and randomly divided into the observation group and the control group,with 51 cases in each group.The patients in the two groups were given routine treatment,oral administration of Metformin Hydrochloride SustainedRelease Tablets and Acarbose Tablets,on this basis,the patients in the observation group were treated with probiotics.Both groups were treated for eight weeks.Results The total clinical effective rate of the observation group was 94.12%,which was significantly higher than 80.39%of the control group(P<0.05).The proportion of iNKT cells and interleukin-4(IL-4)^(+)iNKT in the observation group was significantly higher than that in the control group,the number of bifidobacteria and lactobacillus in the observation group was significantly more than that in the control group,while the proportion of interferon-γ(IFN-γ)^(+)iNKT,the levels of fasting blood glucose(FBG),glycosylated hemoglobin(HbA_(IC)),2-hour postprandial blood glucose(2 hPG),high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6)and lipopolysaccharide(LPS)in the observation group were significantly lower than those in the control group,and the number of enterococci and coccobacteria in the observation group was significantly fewer than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(11.76%vs.17.65%,P>0.05).Conclusion Probiotics can improve the levels of iNKT cells,reduce the level of blood glucose and inflammatory factors,and improve the imbalance of intestinal flora in patients with T2DM.
