川崎病患儿血清Cav-1、PTX-3、RBP4水平与冠状动脉损伤的关系及其预测价值 被引量:4

The Relationship Between Serum Cav-1,PTX-3,RBP4 Levels and Coronary Artery Injury in Children with Kawasaki Disease and Its Predictive Value

摘要 目的探讨川崎病患儿血清陷窝蛋白-1(Cav-1)、正五聚体蛋白-3(PTX-3)、视黄醇结合蛋白4(RBP4)表达水平,分析其与患儿冠状动脉损伤的关系及预测价值。方法选取2015年1月至2019年1月我科收治的川崎病患儿80例(观察组),根据是否合并冠状动脉损伤,分为冠状动脉损伤组(n=32)和冠状动脉正常组(n=48)。同期,另选取我院体检健康儿童50例(对照组)。采用酶联免疫吸附法检测血清Cav-1、PTX-3、RBP4水平。采用多因素Logistic回归分析影响川崎病患儿冠状动脉损伤的危险因素。绘制ROC曲线,分析血清Cav-1、PTX-3、RBP4对川崎病患儿冠状动脉损伤的预测价值。结果观察组血清Cav-1、PTX-3、RBP4水平与对照组相比,差异有统计学意义(P<0.05)。冠状动脉损伤组血清Cav-1、PTX-3、RBP4水平与冠状动脉正常组相比,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,血清NT-proBNP、cTnI、Cav-1、PTX-3、RBP4是川崎病患儿冠状动脉损伤的重要危险因素(P<0.05)。ROC曲线显示,血清Cav-1、PTX-3、RBP4及三者联合检测预测川崎病患儿冠状动脉损伤的AUC分别为0.811、0.759、0.823和0.906。结论血清Cav-1、PTX-3、RBP4水平变化与川崎病的发生发展有关,其可参与川崎病患儿冠状动脉损伤的病理过程,联合检测血清Cav-1、PTX-3、RBP4对评估川崎病患儿冠状动脉损伤有较高临床价值。 Objective To investigate expression levels of serum Cav-1,PTX-3 and RBP4 in children with Kawasaki disease and to analyze its relationship with coronary artery injury and its predictive value.Methods A total of 80 children with Kawasaki disease admitted to our department from January,2015 to January,2019(observation group)were selected for the study.According to whether coronary artery injury was combined,these patients were divided into the coronary artery injury group(n=32)and normal coronary artery group(n=48).During the same period,another 50 healthy children(control group)were selected from physical examination in our hospital.Enzyme-linked immunosorbent assay was conducted to detect serum levels of Cav-1,PTX-3,and RBP4.Multivariate logistic regression was applied to analyze the risk factors of coronary artery injury in children with Kawasaki disease.We drew ROC curve and analyzed the predictive value of serum Cav-1,PTX-3,RBP4 on coronary artery injury in children with Kawasaki disease.Results Serum levels of Cav-1,PTX-3 and RBP4 in the observation group were significantly different from those in the control group(P<0.05).Levels of serum Cav-1,PTX-3 and RBP4 in the coronary artery injury group were significantly different from those in the normal coronary artery group(P<0.05).Multivariate logistic regression analysis showed that serum NT-proBNP,cTnI,Cav-1,PTX-3 and RBP4 were important risk factors for coronary artery injury in children with Kawasaki disease(P<0.05).ROC curve analysis showed that the AUCs of serum Cav-1,PTX-3,RBP4 and the combined detection of the three to predict coronary artery injury were 0.811,0.759,0.823 and 0.906,respectively.Conclusion Changes of serum Cav-1,PTX-3 and RBP4 levels are related to the occurrence and development of Kawasaki disease,and these proteins may participate in the pathological process of coronary artery injury in children with Kawasaki disease.Combined detection of serum Cav-1,PTX-3 and RBP4 has a higher clinical value in assessing coronary artery injury in childr
