右美托咪定对老年患者全麻诱导期血流动力学的影响 被引量:1

Effect of dexmedetomidine on hemodynamics during induction of general anesthesia in elderly patients

摘要 目的观察右美托咪定对老年患者全麻诱导期血流动力学的影响,为其选择更为安全有效的麻醉方法提供参考。方法选择60例择期行全身麻醉的老年患者,随机分为两组,每组30例。观察组(D组)诱导前予以右美托咪定0.5ug/kg泵注,对照组(C组)诱导前予以同等剂量生理盐水。分别记录两组患者入室时(T_(0))、诱导后(T_(1))、插管后(T_(2))的心率、收缩压、舒张压,及呛咳、药物刺激痛、严重心动过缓、低血压等不良反应的发生情况。结果诱导后D组患者SP、DP的下降程度小于C组,插管后C组的SP、DP增加幅度明显高于D组,差异均有统计学意义(P<0.01)。插管后两组的HR有所上升,而C组的HR上升明显,明显高于D组(P<0.01),且高于入室时的基础值(P<0.05)。D组患者发生低血压,需药物干预治疗的情况以及呛咳的发生情况明显少于C组,差异有统计学意义(P<0.05)。结论老年患者全麻诱导期复合使用右美托咪定可有效提高患者诱导期间血流动力学的稳定性,减少麻醉不良反应。 Objective To observe the effect of dexmedetomidine on hemodynamics in elderly patients during general anesthesia induction period,and to provide a reference for the selection of more safe and effective anesthetic methods.Methods Sixty elderly patients were randomly divided into two groups,30 in each group.The observation group(D group)was injected with dexmedetomidine0.5 ug/kg before induction,and the control group(C group)was given the same dose of saline before induction.The heart rate,systolic blood pressure and diastolic blood pressure of the two groups were recorded at the time of entry(T_(0)),after induction(T_(1)),after intubation(T_(2)),and the occurrence of adverse reactions such as cough,drug stimulation pain,severe bradycardia and hypotension were observed.Results The decrease of SP、DP in D group after induction was less than that in C group,and the increase of SP、DP in the C group after intubation was significantly higher than that in the D group(P<0.01).The HR of the two groups increased after intubation,while the HR of the C group increased significantly,which was significantly higher than that of the D group(P<0.01)and higher than the basic value(P<0.05).The incidence of hypotension,drug intervention and cough in D group was significantly lower than that in C group(P<0.05).Conclusion Combined use of dexmedetomidine in elderly patients during general anesthesia induction can effectively improve the stability of hemodynamics and reduce adverse anesthetic reactions during induction.
