产ESBLs大肠埃希菌的感染特点及耐药性分析

摘要 目的:探究产超广谱β-内酰胺酶(ESBLs)大肠埃希菌的感染特点及耐药性。方法:对2019年3月至2020年12月邳州市中医院各科室送检标本中分离出的124株大肠埃希菌进行菌种鉴定。观察这些菌株中产ESBLs大肠埃希菌的检出率,统计这些产ESBLs大肠埃希菌的检出科室分布情况及具体来源。对这些产ESBLs大肠埃希菌进行药敏试验,然后观察其对各类抗菌药的耐药性。结果:1)对124株大肠埃希菌进行菌种鉴定的结果显示,其中有72株产ESBLs大肠埃希菌,检出率为58.06%。2)在72株产ESBLs大肠埃希菌中,检出自神经外科、门诊、肾内科、重症监护室、泌尿外科、外2科、呼吸内科、内分泌科送检标本菌株的占比分别为19.44%、6.94%、11.11%、25.00%、16.67%、11.11%、4.17%、5.56%,其中检出自重症监护室送检标本菌株的占比最高。3)在72株产ESBLs大肠埃希菌中,检出自血液标本、痰液标本、脓液标本、尿液标本、伤口分泌物标本、肿瘤组织标本菌株的占比分别为9.70%、37.50%、6.94%、23.61%、16.67%、5.56%,其中检出自痰液标本菌株的占比最高。4)72株产ESBLs大肠埃希菌对第三代头孢菌素类药物、青霉素类药物及喹诺酮类药物的耐药性较高,其对阿米卡星、舒巴坦、美洛培南、亚胺培南的耐药性较低。结论:与其他科室的患者相比,重症监护室患者发生产ESBLs大肠埃希菌感染的风险较高。与其他标本相比,痰液标本中更易检出产ESBLs大肠埃希菌。产ESBLs大肠埃希菌对第三代头孢菌素类药物、青霉素类药物及喹诺酮类药物的耐药性较高,对阿米卡星、舒巴坦、美洛培南、亚胺培南的耐药性较低。 Objective:To investigate the infection characteristics and drug resistance of escherichia coli producing ultrabroad spectrumβ-lactamase(ESBLs).Methods:124 strains of Escherichia coli isolated from pizhou Hospital of Traditional Chinese Medicine from March 2019 to December 2020 were identified.The detection rate of ESBLs-producing EScherichia coli in these strains was observed,and the department distribution and specific sources of esBLs-producing EScherichia coli were counted.These ESBLs-producing EScherichia coli were subjected to drug sensitivity tests,and their resistance to various antibacterial drugs was observed.Results:1)Among 124 strains of EScherichia coli,72 strains were esBLs-producing escherichia coli,with a detection rate of 58.06%.2)Among 72 ESBL-producing EScherichia coli strains,The proportions of strains from neurosurgery,outpatient department,nephrology department,INTENSIVE care unit,urology department,2 departments of external surgery,respiratory department and endocrinology department were 19.44%,6.94%,11.11%,25.00%,16.67%,11.11%,4.17%and 5.56%,respectively.Among them,strains from the intensive care unit accounted for the highest proportion.3)Among the 72 ESBLS-producing EScherichia coli strains,the proportions of blood samples,sputum samples,pus samples,urine samples,wound secretion samples and tumor tissue samples were 9.70%,37.50%,6.94%,23.61%,16.67%and 5.56%,respectively,and the proportions of sputum samples were the highest.4)The 72 ESBLs-producing EScherichia coli strains showed high resistance to third-generation cephalosporins,penicillins and quinolones,but low resistance to amikacin,sulbactam,melopenem and imipenem.Conclusions:Patients in intensive care units have a higher risk of esBLs-producing EScherichia coli infection than patients in other units.Compared with other specimens,sputum specimens were easier to detect ESBLs-producing EScherichia coli.Esbls-producing EScherichia coli had higher resistance to third-generation cephalosporins,penicillins and quinolones,but lower resista
