摘要
目的探讨三甲医院儿科2013—2019年大肠埃希菌感染的分布与耐药性变迁,为临床治疗大肠埃希菌感染提供参考。方法采用法国生物梅里埃公司的VITEK2-compact全自动微生物仪对2013年1月至2019年12月中山市博爱医院儿科住院患儿送检的痰液、中段尿、血液等各类标本进行细菌培养及菌株鉴定,选取各年度分离的产超广谱β-内酰胺酶(ESBLs)大肠埃希菌的感染分布特点进行分析;采用K-B纸片扩散法对氨苄西林、头孢唑林、头孢噻肟等18种抗菌药物进行药敏试验,统计2013—2019年各年度产ESBLs大肠埃希菌的耐药结果。结果2013—2014年产ESBLS大肠埃希菌的检出率上升了9%,2015—2016年检出率下降10%,2017—2019年逐年平稳下降,标本主要来自于痰液、中段尿,对阿米卡星、亚胺培南、派拉西林/他唑巴坦、头孢替坦、美罗培南较为敏感。结论加强对大肠埃希菌感染分布及耐药性变迁的分析对于指导临床合理应用抗菌药物具有重要价值。
Objective To explore the distribution and drug resistance changes of escherichia coli infection in pediatrics of 3A hospital from 2013 to 2019,and to provide reference for clinical treatment of escherichia coli infection.Methods The VITEK2-compact automatic microbiometer of French BioMérieux was used to conduct bacterial culture and culture of various specimens of sputum,mid-section urine,blood and other specimens submitted by hospitalized children in the Pediatric Department of Zhongshan Boai Hospital from January 2013 to December 2019.The infection distribution characteristics of the extended-spectrumβ-lactamase(ESBLs)-producing Escherichia coli isolated in each year were selected for analysis;the KB paper diffusion method was used to analyze 18 antibacterial drugs such as ampicillin,cefazolin,and cefotaxime to carry out drug susceptibility tests,and results of drug resistance of ESBLs-producing Escherichia coli was counted each year from 2013 to 2019.Results The detection rate of ESBLs Escherichia coli increased by 9%from 2013 to 2014,decreased by 10%from 2015 to 2016,and decreased steadily from 2017 to 2019.The samples mainly came from sputum and midstream urine,and were sensitive to amikacin,imipenem,piperacillin/tazobactam,cefotetan and meropenem.Conclusion To strengthen the analysis of the distribution of E.coli infection and the change of drug resistance is of great value to guide the clinical rational use of antibiotics.
