摘要
目的研究Ⅲ类固定磁力功能矫治器矫治后颞下颌关节的改建情况。方法选择15例矫治前经X线头影测量分析,诊断为功能性和骨性Ⅲ类错[牙合]畸形的患者为研究对象,采用Ⅲ类固定磁力功能矫治器进行治疗,在治疗前后使用MRI评估颞下颌关节的变化。结果治疗后发现关节前间隙增大,后间隙减小,关节间隙指数减小,髁突在关节窝内的位置发生向后移位;盘髁角、前带盘髁角增大,关节盘在生理范围内前移位,关节盘前带位置的变化与后带位置的变化一致;髁突头角增大,髁突前屈;节窝后斜面角和关节窝角,差异不显著。结论固定磁力功能矫治器治疗后生长期Ⅲ类错[牙合]畸形患者的颞下颌关节发生了适应性改变,固定磁力功能矫治器矫治后髁突向后移位,关节盘前移位,双板区厚度减小,髁突头前屈。
Objective To study the remodeling of temporomandibular joint after treatment with ClassⅢfixed magnetic functional appliance.Methods Fifteen patients diagnosed with functional and bony ClassⅢmalocclusion by X-ray cephalometric analysis were treated with ClassⅢfixed magnetic functional appliance.MRI was conducted before and after treatment to assess changes of temporomandibular joint.Results After orthopedic treatment,the anterior space of joint increased;the posterior space of it decreased;the joint space index decreased,and the position of condyle process in the articular fossa was shifted backward.The disc condyle angle and the anterior band disc condylar angle increased.The joint disc moved backward within the physiological range,and the change in anterior band position of the joint disc was consistent with that in the position of its posterior band.The angle of condylar process head increased,and the condyle process flexed forward.There was no significant difference between the angle of articular fossa posterior bevel and the angle of articular fossa.Conclusion Fixed magnetic functional appliance treatment can induce adaptive changes within the physiological range in TMJ of patients with ClassⅢmalocclusion during the growing period.After correction using fixed magnetic functional appliance,the posterior condylar process is displaced backward;the articular disc is displaced forward;the thickness of bilaminar area is reduced,and the condylar process head is flexed forward.
