摘要
目的探讨比较乳牙期应用MRC与功能性矫治器(FR-Ⅲ)矫治安氏Ⅲ类错颌的临床效果。方法56例乳牙期安氏Ⅲ类错颌患儿,随机分为FR-Ⅲ组与MRC组,各28例。FR-Ⅲ组应用FR-Ⅲ功能矫形器,MRC组应用I3系列矫形器,配合肌功能训练。比较两组治疗前后颅底-上牙槽座角(∠SNA)、颅底-下牙槽座角(∠SNB)、上下牙槽座角(∠ANB)、上中切牙-前颅底平面角(∠U1-SN)、下中切牙长轴与NB连线之下交角(∠L1-NB)。结果治疗前,FR-Ⅲ组∠SNA(78.31±1.72)°、∠SNB(79.64±1.73)°、∠ANB(-2.00±0.87)°、∠U1-SN(104.07±1.07)°、∠L1-NB(23.71±1.06)°;MRC组∠SNA(78.08±0.98)°、∠SNB(79.71±0.99)°、∠ANB(-1.85±0.66)°、∠U1-SN(103.78±1.62)°、∠L1-NB(24.35±1.54)°。治疗后,FR-Ⅲ组∠SNA(79.54±1.39)°、∠SNB(78.28±1.38)°、∠ANB(1.21±0.80)°、∠U1-SN(105.14±1.41)°、∠L1-NB(22.64±1.22)°;MRC组∠SNA(78.12.±1.35)°、∠SNB(78.85±1.16)°、∠ANB(-0.64±0.84)°、∠U1-SN(104.85±1.70)°、∠L1-NB(22.14±1.83)°。治疗后,两组患儿∠SNB、∠ANB、∠U1-SN、∠L1-NB均优于本组治疗前,FR-Ⅲ组∠SNA大于本组治疗前,差异均具有统计学意义(P<0.05)。MRC组∠SNA与本组治疗前比较,差异无统计学意义(P>0.05)。FR-Ⅲ组平均疗程(6.8±0.5)个月短于MRC组的(7.9±0.8)个月,差异具有统计学意义(P<0.05)。结论MRC矫治器和FR-Ⅲ矫治器都有临床效果,能够解除临床反颌,舒适度方面FR-Ⅲ不如MRC,但唇挡和颊屏能够拉开唇颊肌肉,更好促进上颌骨发育。
Objective To discuss the clinical effect of MRC and functional appliance(FR-Ⅲ)in the treatment of Angle ClassⅢmalocclusion in deciduous dentition.Methods A total of 56 children with ClassⅢmalocclusion in deciduous dentition were randomly divided into FR-Ⅲgroup and MRC group,with 28 cases in each group.FR-Ⅲgroup was treated with FR-Ⅲfunctional appliance,and MRC group was treated with I3 series orthosis cooperated with muscle function training.The skull base-upper alveolar seat angle(∠SNA),skull baselower alveolar seat angle(∠SNB),upper and lower alveolar seat angle(∠ANB),upper central incisor-anterior skull Bottom plane angle(∠U1-SN),the angle of intersection between the long axis of the lower central incisor and NB(∠L1-NB)before and after treatment of the two groups were compared.Results Before treatment,∠SNA,∠SNB,∠ANB,∠U1-SN and∠L1-NB of FR-Ⅲgroup were(78.31±1.72)°,(79.64±1.73)°,(-2.00±0.87)°,(104.07±1.07)°and(23.71±1.06)°,which were(78.08±0.98)°,(79.71±0.99)°,(-1.85±0.66)°,(103.78±1.62)°and(24.35±1.54)°of MRC group.After treatment,∠SNA,∠SNB,∠ANB,∠U1-SN and∠L1-NB of FR-Ⅲgroup were(79.54±1.39)°,(78.28±1.38)°,(1.21±0.80)°,(105.14±1.41)°and(22.64±1.22)°,which were(78.12.±1.35)°,(78.85±1.16)°,(-0.64±0.84)°,(104.85±1.70)°and(22.14±1.83)°of MRC group.After treatment,∠SNB,∠ANB,∠U1-SN and∠L1-NB of the two groups were better than those before treatment,and∠SNA of FR-Ⅲgroup was greater than that before treatment,and the difference was statistically significant(P<0.05).In MRC group,there was no statistically significant difference in∠SNA after treatment compared with that before treatment(P>0.05).The average course of treatment in FR-Ⅲgroup was(6.8±0.5)months,which was shorter than(7.9±0.8)months of MRC group,and the difference was statistically significant(P<0.05).Conclusion Both the MRC appliance and the FR-Ⅲappliance have clinical effect and can relieve clinical reverse jaw.FR-Ⅲis not as comfortable as MRC,but lab
