摘要
钙化性主动脉瓣疾病(calcific aortic valve disease,CAVD)为本虚标实、虚实夹杂之证。本虚以脏腑虚损为主,标实则以瘀毒、痰浊、寒凝、气滞为主。中医药治疗钙化性主动脉瓣疾病主要包括中药复方治疗和中药单体治疗。其中,中药复方治疗包括活血化瘀通络、化痰泄浊调脂、调补气血阴阳等;中药单体治疗包括王不留行黄酮苷、丹参酮ⅡA、淫羊藿素、三七总皂苷、黄连素等。目前,国内尚缺乏有关CAVD前瞻性、多中心的大规模流行病学调查,CAVD发病的具体机制尚不明确,探讨其类型和构成的文献较少,辨证分型尚无统一标准,临床研究样本量偏少,尚缺乏循证研究的充足数据。今后,应发挥中医药防治复杂疾病的优势,扩大样本量,积极开展相关的基础及临床研究,制定统一的辨证分型标准,深入探讨该病的发病机制,为中医药治疗CAVD提供更加明确的理论依据和临床证据。
Calcified aortic valve disease(CAVD)is the syndrome of deficient root cause and excessive manifestations with deficiency-excess in complexity.The deficient root cause is mainly manifested as the deficiency of zang-fu organs,while excessive manifestations mainly include blood stasis,phlegm turbidity,cold coagulation and qi stagnation.TCM treatments of CAVD mainly include Chinese medicinal compound treatment and Chinese medicinal monomer treatment.Among them,Chinese medicinal compound treatment includes promoting blood circulation and removing blood stasis and dredging collaterals,resolving phlegm and relieving turbidity and regulating lipid,regulating qi and blood and yin and yang,etc.Chinese medicinal monomer treatment includes flavonoid glycosides of Astragalus chinensis,tanshinoneⅡA,icariin,panax notoginseng saponins,berberine,etc.At present,there is still a lack of prospective and multi-center large-scale epidemiological investigation on CAVD in China;the specific mechanism of CAVD is not clear;there are few articles to explore its type and composition;there is no unified standard for syndrome differentiation;the sample size of clinical research is small,and there is still a lack of sufficient data for evidencebased research.In the future,it is necessary to give full play to the advantages of TCM in the prevention and treatment of complex diseases,expand the sample size,actively carry out relevant basic and clinical research,formulate unified syndrome differentiation and typing standards,and deeply explore the pathogenesis of the disease,so as to provide more clear theoretical basis and clinical evidence for the treatment of CAVD with TCM.
