摘要
目的探讨循环肿瘤血管内皮细胞(CTEC)检测联合放大内镜检查对早期胃癌的诊断价值。方法选取160例胃部病变患者,均给予CTEC检测和放大内镜检查,比较早期胃癌患者与胃部良性病变患者的CTEC检出数和阳性检出率。以病理检查结果作为诊断金标准,评估CTEC和放大内镜单独及联合检查对早期胃癌的诊断效能。结果病理学检查结果显示,早期胃癌80例,胃良性病变80例。CTEC检测结果显示,早期胃癌患者CTEC检出个数和阳性检出率均明显高于胃良性病变患者(P﹤0.01)。受试者工作特征(ROC)曲线显示,CTEC检出数诊断早期胃癌的曲线下面积(AUC)为0.812,此时的灵敏度为62.50%,特异度为98.80%,最佳截断值为CTEC检出数≥5个;当最佳截断值定为6个时,CTEC检出数诊断早期胃癌的特异度为100%(此时AUC为0.710)。放大内镜检查诊断早期胃癌的灵敏度为95.00%、特异度为62.50%、准确度为78.75%、阳性预测值为71.70%、阴性预测值为92.59%。CTEC联合放大内镜诊断早期胃癌的灵敏度为97.50%、特异度为85.00%、准确度为89.02%、阳性预测值为86.67%、阴性预测值为97.14%。结论CTEC检测联合放大内镜检查对早期胃癌的诊断效能较高,且均高于二者单独检测,对提高早期胃癌的诊断有一定的临床意义。
Objective To explore the diagnostic value of circulating tumor vascular endothelial cell(CTEC)com-bined with magnifying endoscopy in the diagnosis of early gastric cancer.Method A total of 160 patients with gastric le-sions were selected and given CTEC detection and magnifying endoscopy.The number and positive detection rate of CTEC were compared between patients with early gastric cancer and patients with benign gastric lesions.Using pathologi-cal findings as the gold standard for diagnosis,evaluated the diagnostic efficacy of CTEC and intrascale alone and in com-bination for early gastric cancer.Result Pathological examination showed 80 cases of early gastric cancer and 80 cases of benign gastric lesions.The CTEC detection results showed that the number and positive rate of CTEC in patients with early gastric cancer were significantly higher than those in patients with gastric benign disease(P<0.01).The receiver op-erating characteristic(ROC)curve showed that the area under the curve(AUC)of CTEC cells detected in the diagnosis of early gastric cancer was 0.812,with sensitivity of 62.50%,specificity of 98.80%and optimal cut-off value of≥5.When the optimal cut-off value was set at 6,the specificity of CTEC cell detection for early gastric cancer was 100%(AUC was 0.710 at this time).The sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 95.00%,62.50%,78.75%,71.70%and 92.59%respectively.The sensitivity,specificity and accuracy of CTEC combined with magnifying endoscopy in the diagnosis of early gastric cancer were 97.50%,85.00%,89.02%,86.67%and 97.14%respectively.Conclusion CTEC combined with magnifying endoscopy has higher diagnostic efficacy in early gastric cancer,and both of them are higher than the two detection alone,which has certain clinical significance in improving the diagnosis of early gastric cancer.
