摘要
目的:分析艾滋病(AIDS)合并肝细胞癌(HCC)患者的病理特征,从病理角度探讨HIV感染对艾滋病合并肝细胞癌生存率的影响,寻找PD-1/PD-L1抑制剂治疗肝细胞癌合并艾滋病的可能依据。方法:回顾性分析2013年1月-2018年12月广州医科大学附属市八医院接受根治性肝切除术的26例HCC合并艾滋病患者(HIV+组)的临床及病理资料,加做相关免疫组化检查,与同期接受根治性肝切除术的113例非艾滋病HCC患者(HIV-组)对比。结果:HIV+组微血管侵犯(MVI)发生率高于HIV-组,差异有统计学意义(P=0.017),HIV+组肿瘤直径>5 cm比例高于HIV-组(P=0.019)。两组PD-1、PD-L1阳性表达率比较差异均无统计学意义(P>0.05),单因素分析PD-1、PD-L1表达与预后不相关。结论:HCC+艾滋病患者预后更差,可能与MVI有关。肿瘤大小是影响预后的重要因素。PD-L1抑制剂应用于HCC+艾滋病应该被考虑。
Objective:To analyze the pathological features of acquired immune deficiency syndrome(AIDS)complicated with hepatocellular carcinoma(HCC)patients,to explore the effect of HIV infection on the survival rate of AIDS complicated with HCC from the pathological perspective,and to search for the possible basis of PD-1/PD-L1 inhibitor in the treatment of HCC complicated with AIDS.Method:The clinical and pathological data of 26 HCC complicated with AIDS(HIV+group)who underwent radical hepatectomy in Guangzhou Eighth People’s Hospital,Guangzhou Medical University from January 2013 to December 2018 were retrospectively analyzed.Additional immunohistochemical examination were done.Compared with 113 HCC patients without AIDS(HIV-group)who underwent radical hepatectomy at the same time.Result:The incidence of microvascular invasion(MVI)in HIV+group was higher than that in HIV-group,and the difference was statistically significant(P=0.017);the proportion of tumor diameter>5 cm in HIV+group was higher than that in HIV-group(P=0.019).There were no significant differences in the positive expression rate of PD-1,PD-L1 between the two groups(P>0.05),PD-1,PD-L1 expression were not correlated with prognosis by univariate analysis.Conclusion:HCC+AIDS patients have worse prognosis,which may owe to higher possibility of MVI.Tumor size is also an influencing factor.PD-L1 inhibitors in HCC+AIDS should be considered.
