摘要
目的探讨Ang-1、Ang-2及Ang-1/Ang-2比值与重症肺炎患儿病情严重程度的相关性。方法对收治的小儿重症肺炎患儿113例,检测其血清Ang-1、Ang-2水平,计算Ang-1/Ang-2比值;根据患儿预后情况分为预后良好组和预后不良组,用多因素Logistic回归方程分析影响重症肺炎患儿预后不良的独立危险因素;Spearman线性方程分析血清Ang-1、Ang-2及Ang-1/Ang-2比值与APCHE-Ⅱ评分的相关性;ROC曲线分析Ang-1、Ang-2及Ang-1/Ang-2比值在早期预测重症肺炎患儿预后的价值。结果预后不良组在APACHE-Ⅱ评分、血清PCT、CRP水平以及Ang-1、Ang-2水平高于预后良好组,年龄、Ang-1/Ang-2比值低于预后良好组(P<0.05);高APACHE-Ⅱ评分(OR=1.383)、高Ang-2水平(OR=2.082)、低Ang-1/Ang-2比值(OR=0.009)以及高PCT(OR=2.205)水平是重症肺炎患儿预后不良的独立危险因素(P<0.05);相关性分析显示,Ang-1/Ang-2比值与APACHE-Ⅱ评分呈负相关性(ρ=-0.509,P<0.05),Ang-2水平与APACHE-Ⅱ评分呈正相关性(ρ=0.611,P<0.05);ROC曲线显示,Ang-1/Ang-2比值在早期预测重症肺炎患儿预后不良的AUC为0.867,敏感度为95.83%,特异度为75.78%。结论早期检测血清Ang-1、Ang-2水平有助于评估重症肺炎患儿的严重程度,并可评估预后。
Objective To explore the correlation between Ang-1,Ang-2 and Ang-1/Ang-2 ratios and the severity of children with severe pneumonia.Methods 113 children with severe pneumonia were enrolled.The serum Ang-1 and Ang-2 levels of the children were detected,and the Ang-1/Ang-2 ratio was calculated.According to the prognosis of children,the patients were divided into good prognosis group and poor prognosis group.Multivariate logistic regression equation was used to analyze independent risk factors affecting the poor prognosis of children with severe pneumonia.The correlation between serum Ang-1,Ang-2,Ang-1/Ang-2 ratio and APCHE-Ⅱscore were analyzed by using Spearman linear equation analysis;The value of Ang-1,Ang-2 and Ang-1/Ang-2 ratio in early prediction of the prognosis of children with severe pneumonia were calculated by using ROC curve analysis.Results In the poor prognosis group,the APACHE-Ⅱscore,serum PCT,CRP levels,and Ang-1 and Ang-2 levels were higher than those in the good prognosis group,while the age and Ang-1/Ang-2 ratio were lower than those in the good prognosis group(P<0.05).High APACHE-Ⅱscore(OR=1.383),high Ang-2 level(OR=2.082),low Ang-1/Ang-2 ratio(OR=0.009)and high PCT(OR=2.205)level were independent risk factors for poor prognosis children with severe pneumonia(P<0.05).Correlation analysis showed that Ang-1/Ang-2 ratio was negatively correlated with APACHE-Ⅱscore(ρ=-0.509,P<0.05),and Ang-2 level was positively correlated with APACHE-Ⅱscore(ρ=0.611,P<0.05).The ROC curve showed that the AUC of the Ang-1/Ang-2 ratio in early predicting poor prognosis of children with severe pneumonia was 0.867,the sensitivity was 95.83%and the specificity was 75.78%.Conclusion Early detection of serum Ang-1 and Ang-2 levels can help the doctor to assess the severity of children with severe pneumonia and predict the prognosis.
