摘要
目的:比较^(68)Ga-PSMA PET/CT与^(18)F-FDG PET/CT在肾透明细胞癌术前和术后复发及转移中的探测价值。方法 :回顾性纳入2018年8月—2020年12月在我院行^(68)Ga-PSMA PET/CT和^(18)F-FDG PET/CT检查的肾透明细胞癌患者15例(初诊9例,术后6例)。将患者全身病变按部位分类:原发肿瘤、局部复发、静脉侵犯、肾上腺转移、淋巴结转移、肺转移、骨转移、肌肉转移和其它转移。以病理和随访结果为标准,比较两种PET示踪剂的阳性探测率。并分别在两种PET/CT影像上测量患者不同类别病灶的最大标准摄取值(Maximum standardize uptake value,SUVmax),用统计学方法比较各类病变在^(68)Ga-PSMA PET/CT和^(18)F-FDG PET/CT中SUVmax值的差别。结果:^(68)Ga-PSMA和^(18)F-FDG PET/CT探测肾透明细胞癌淋巴结转移的阳性率分别为100%和87.5%,两种显像剂探测原发肿瘤、局部复发、静脉侵犯、肾上腺转移、淋巴结转移、肺转移、骨转移、肌肉转移和肝转移的阳性率均为100%。肾透明细胞癌原发肿瘤、局部复发、淋巴结转移、骨转移灶的SUVmax在^(68)Ga-PSMA PET/CT上更高,有显著统计学差异(P<0.05)。肺转移、肾上腺转移、肌肉转移病灶的SUVmax在两种显像剂间无统计学差异(P>0.05)。结论:肾透明细胞癌原发肿瘤、局部复发、淋巴结转移和骨转移的SUVmax在^(68)Ga-PSMA PET/CT上更高。^(68)Ga-PSMA PET/CT在肾透明细胞癌术前和术后复发及转移探测方面显示出了良好的临床应用潜力。
Objective: To compare the diagnostic performance of^(68)Ga-PSMA and^(18)F-FDG PET/CT in patients with suspected primary or recurrence of clear cell renal cell carcinoma(ccRCC). Methods: Fifteen patients with ccRCC(9 primary, 6 recurrent) who had undergone both^(68)Ga-PSMA and^(18)F-FDG PET/CT were retrospectively enrolled. Lesions were classified by their localization(primary tumor, local relapse, vascular invasion, adrenal, lymph nodes, lung, bones, muscles and other metastases). The positive rates of the PET/CT scans in the involved lesions and the uptake values were compared. Results:^(68)Ga-PSMA PET/CT had a slightly higher positive rate than^(18)F-FDG PET/CT in detecting lymph nodes metastases(100% vs.87.5%). The positivity rate was consistently high for^(68)Ga-PSMA-PET/CT and^(18)F-FDG-PET/CT in detecting primary tumor, local relapse, vascular invasion and lesions in the adrenal, lung, bones, muscles and liver(all 100%).^(68)Ga-PSMA also demonstrated significantly higher uptake than^(18)F-FDG in the matched disease in the primary tumor, local relapse, lymph nodes and bone metastases(P<0.05). However, the uptake value of metastases to adrenal, lung, and muscles was found to have no significant difference(P>0.05). Conclusions: Primary tumor, local relapse, lymph nodes and bone metastases show significantly higher uptake of^(68)Ga-PSMA than^(18)F-FDG.^(68)Ga-PSMA PET/CT might be a useful imaging agent for the assessment of ccRCC.
