摘要
目的观察不同诊断方法对乳腺肿块早期的诊断与鉴别价值。方法选取2020年5月-2021年5月我院诊治的62例早期乳腺肿块患者为研究对象,均采用常规超声和超声引导下粗针穿刺活检检查,以病理诊断结果为诊断标准,比较常规超声和超声引导下粗针穿刺活检检出率、漏诊和误诊率、诊断效能,并分析良恶性肿瘤超声诊断图像特征。结果超声引导下粗针穿刺活检检出率为93.55%,高于常规超声的80.65%,差异有统计学意义(P<0.05);超声引导下粗针穿刺活检误诊和漏诊率为6.45%,低于常规超声的19.35%,差异有统计学意义(P<0.05);超声引导下粗针穿刺活检灵敏度、特异性、阳性预测值、阴性预测值均高于常规超声,差异有统计学意义(P<0.05);超声诊断良性肿瘤形态规则、边缘光滑、边界回声清晰、内部回声分布均匀、声衰减不明显、侧后声影明显、纵横比例<0.7检出率均高于恶性肿瘤,差异有统计学意义(P<0.05)。结论早期乳腺肿块多数可表现出明显超声征象特征,常规超声与超声引导下粗针穿刺活检对乳腺肿块早期的诊断与鉴别均具有一定价值,但超声引导下粗针穿刺活检可提高乳腺肿块早期的诊断率、灵敏度、特异性。
Objective To observe the value of different diagnostic methods for early breast masses.Methods A total of 62 patients with early breast masses diagnosed and treated in our hospital from May 2020 to May 2021 were selected as the research objects.All patients were examined by conventional ultrasound and ultrasound-guided core needle biopsy.Taking the pathological diagnosis results as the diagnostic criteria,the detection rate,missed diagnosis and misdiagnosis rate,and diagnostic efficacy of conventional ultrasound and ultrasound-guided core needle biopsy were compared,and the characteristics of ultrasound diagnostic images of benign and malignant tumors were analyzed.Results The detection rate of ultrasound-guided core needle biopsy was 93.55%,which was higher than 80.65%of conventional ultrasound,and the difference was statistically significant(P<0.05).The misdiagnosis and missed diagnosis rate of ultrasound-guided core needle biopsy was 6.45%,which was lower than 19.35%of conventional ultrasound,and the difference was statistically significant(P<0.05).The sensitivity,specificity,positive predictive value and negative predictive value of ultrasound-guided core needle biopsy were higher than those of conventional ultrasound,and the difference was statistically significant(P<0.05).The detection rate of benign tumors by ultrasound diagnosis with regular shape,smooth edge,clear boundary echo,uniform internal echo distribution,no obvious acoustic attenuation,obvious posterior acoustic shadow,longitudinal and transverse ratio<0.7 were higher than those of malignant tumors,the difference was statistically significant(P<0.05).Conclusion Most of the early breast masses can show obvious ultrasonic signs.Conventional ultrasound and ultrasound-guided core needle biopsy have certain value in the diagnosis and identification of early breast masses,but ultrasound-guided needle biopsy can improve the diagnostic rate,sensitivity and specificity of early breast masses.
