摘要
目的:研究连续静脉-静脉血滤对尿毒症患者的相关影响;方法:将2018年6月至2020年12月期间在我院肾病内科规律治疗的73例尿毒症患者作为研究对象,进行连续静脉-静脉血滤(continuous venous hemofiltration,CVVH)治疗。分别对这些患者治疗前、治疗24h后及治疗72h后肾功能、凝血功能、血流动力学、炎症因子水平的变化进行监测和分析,研究连续静脉-静脉血滤对尿毒症患者的相关影响。结果:治疗后患者的BUN、SCr的水平均明显下降,且患者治疗后72h的BUN、SCr的水平明显要低于治疗后24小时(P<0.05)。治疗后,患者的PLT、FIB下降,且治疗后72h明显低于治疗后24h(P<0.05);患者的PT、APTT比治疗前明显延长,且治疗后72h的PT、APTT明显比治疗后24h更加延长(P<0.05)。治疗后患者的HR、PCWP明显下降,且治疗后72h明显低于治疗后24h(P<0.05);而MAP明显回升,治疗后72h明显高于治疗后24h(P<0.05)。治疗后患者的HR、PCWP明显下降,且治疗后72h明显低于治疗后24h(P<0.05)。结论:CVVH治疗过程中对患者的肾功能、血流动力学、凝血功能都具有良好的改善作用,还可以很大程度上清除患者体内炎症因子,从而提高免疫功能,延缓疾病进展,值得临床借鉴。
Objective:To study the impact of continuous venous-venous hemofiltration on patients with uremia.Methods:From June 2018 to December 2020,73 patients with uremia who were regularly treated in the Department of Nephrology in our hospital were selected as the research object for continuous venous-venous hemofiltration(CVVH)treatment.The changes in renal function,coagulation function,hemodynamics,and inflammatory factor levels in these patients before treatment,24h after treatment and 72h after treatment were monitored and analyzed.To study the impact of continuous venous-venous hemofiltration on patients with uremia.Results:After treatment,the levels of BUN and SCr of patients decreased significantly,and the levels of BUN and SCr of patients 72h after treatment were significantly lower than 24 hours after treatment(P<0.05).After treatment,the patient’s PLT and FIB decreased,and 72h after treatment was significantly lower than 24h after treatment(P<0.05);The patient’s PT and APTT were significantly longer than before treatment,and the PT and APTT at 72 hours after treatment were significantly longer than 24 hours after treatment(P<0.05);After treatment,the patient’s HR and PCWP decreased significantly,and 72h after treatment was significantly lower than 24h after treatment(P<0.05);After treatment,the patient’s HR and PCWP decreased significantly,and 72h after treatment was significantly lower than 24h after treatment(P<0.05).Conclusion:During CVVH treatment,it can improve the renal function,hemodynamics,and coagulation function of the patient.It can also eliminate the inflammatory factors in the patient to a large extent,thereby improving the immune function and delaying the progression of the disease.It is worthy of clinical reference.
