摘要
目的探讨精细化专科护理对老年2型糖尿病合并肺部感染病人病情变化及转归的影响。方法回顾性分析因肺部感染入住感染科病房的老年糖尿病病人,收集病人的一般资料以及临床特征资料,包括血常规,CRP、肝素结合蛋白、降钙素原等炎症因子指标,以及病人的住院时间和预后等信息,并比较采用一般常规专科护理以及接受精细化专科护理的老年病人炎症因子、感染指标以及预后的差异。结果入院时,2组病人性别、年龄、HbA1c、合并高血压、吸烟、饮酒等一般资料比较,差异无统计学意义。与治疗前比较,2组干预治疗1周后,WBC、中性粒细胞计数、淋巴细胞计数、单核细胞计数差异亦无统计学意义。入院时,2组CRP、肝素结合蛋白、降钙素原水平差异无统计学意义;干预后,2组上述指标水平均较入院时降低,且精细化专科护理组CRP及肝素结合蛋白降低更明显。精细化专科护理组住院时间短于一般常规护理组(P<0.05),2组治疗满意率及治愈率差异无统计学意义(P>0.05)。结论精细化专科护理可协同医疗措施进一步降低糖尿病合并肺部感染病人体内炎症应激水平,缩短住院时间,可应用于老年糖尿病合并肺部感染病人。
Objective To investigate the effect of refined specialist nursing on inflammatory factors in the elderly patients with diabetes complicated with pulmonary infection.Methods A total of 35 elderly patients with diabetes complicated with pulmonary infection were retrospectively analyzed.They were divided into refined specialist nursing group and routine nursing group by admission order.The clinical data including blood examination,inflammation factors and hospital stay were recorded and compared between the two groups.Results There were no significant differences in sex,age,glycosylated hemoglobin,hypertension,smoking,drinking,the levels of C-reactive protein(CRP),heparin binding protein(HBP),procalcitonin(PCT)between the two groups.Compared with admission,there were no significant differences in the levels of white blood cell,neutrophil,lymph cell,mononuclear cell one week later in the two groups.After intervention,the levels of CRP,HBP of the two groups were significant decreased than those at admission,especially in refined specialist nursing group.The hospital stay of refined specialist nursing group was shorter than that of routine nursing group(P<0.05).Conclusions Refined specialist nursing can reduce the level of inflammation stress of elderly patients with diabetes complicated with pulmonary infection,shorten hospital stay,which is worthy of application.
