咽拭子培养联合降钙素原、C反应蛋白及肺炎支原体检测在儿童肺炎诊断中的应用 被引量:3

摘要 目的探讨咽拭子培养联合降钙素原(PCT)、C反应蛋白(CRP)及肺炎支原体(MP)检测在儿童肺炎分型诊断中的应用。方法选取开封市人民医院2018年10月至2020年11月儿科病区收治住院的肺炎患儿339例作为观察对象,其中细菌性肺炎115例、病毒性肺炎113例、肺炎支原体肺炎111例,另外选取健康儿童105例设为对照,所有标本进行全血PCT、CRP和MP检测,同时进行MP培养采集咽拭子标本,对所有结果进行比较。结果血清PCT和CRP在细菌性肺炎组中均显著上升(P<0.05);且PCT+CRP联合检测针对细菌性肺炎的诊断与鉴别明显优于单独检测(P<0.05)。肺炎支原体肺炎组MP检测阳性率、咽拭子培养阳性率显著高于其他几组(P<0.05);且MP+咽拭子培养联合检测针对肺炎支原体肺炎的诊断与鉴别明显优于单独检测(P<0.05)。结论咽拭子MP培养联合MP检测能更精准鉴别儿童肺炎支原体肺炎;CRP联合PCT检测对于准确地区分儿童型细菌性肺炎具有鉴别意义;咽拭子培养联合PCT、CRP及MP检测对儿童肺炎的分型有很高的应用价值。 【Objective】To explore the application of pharyngeal swab culture combined with procalcitonin(PCT),C-reactive protein(CRP)and Mycoplasma pneumoniae(MP)detection in the diagnosis of pediatric pneumonia.【Methods】A total of 339 children with pneumonia admitted to the pediatric ward of Kaifeng People's Hospital from October 2018 to November 2020 were selected as the observation objects,including 115 cases of bacterial pneumonia,113 cases of viral pneumonia and 111 cases of Mycoplasma pneumoniae pneumonia.Another 105 healthy children were selected as the control group.All samples were tested for PCT,CRP and MP in whole blood,and Mycoplasma pneumoniae culture was carried out at the same time.Swab samples were collected and all results were compared.【Results】The expression level of PCT and CRP in bacterial pneumonia group was significantly higher than that in non bacterial pneumonia group(P<0.05);the combined detection of PCT and CRP was significantly better than the single detection in the diagnosis and differentiation of bacterial pneumonia(P<0.05).The positive rates of MP detection and throat swab culture in Mycoplasma pneumoniae pneumonia group were significantly higher than those in other groups(P<0.05);the combined detection of MP and throat swab culture was significantly better than the single detection in the diagnosis and differentiation of Mycoplasma pneumoniae pneumonia(P<0.05).【Conclusion】Pharyngeal swab Mycoplasma pneumoniae culture combined with MP detection can more accurately identify children with Mycoplasma pneumoniae pneumonia;CRP combined with PCT detection has differential significance for accurately distinguishing children with bacterial pneumonia;pharyngeal swab culture combined with PCT,CRP and MP detection has high application value for the classification of children with pneumonia.
