摘要
目的研讨阶梯式康复模式对腰椎间盘突出症(LDH)患者腰椎功能、术后疼痛的影响。方法选取2019年2月至2021年2月广州市中西医结合医院手术治疗的100例LDH患者,按随机数字表法分为两组,每组各50例。对照组实施常规护理指导,观察组开展阶梯式康复模式干预,比较两组的护理满意度、术后并发症发生情况及不同干预阶段腰椎功能与疼痛程度。结果术后第4、12周,观察组的日本骨科协会评估表(JOA)评分高于对照组,Oswestry功能障碍指数及视觉模拟评分法(VAS)评分低于对照组,差异均有统计学意义(P<0.05)。观察组干预后的总满意度高于对照组,差异有统计学意义(P<0.05)。观察组的术后发生神经根水肿、椎间隙感染等并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论阶梯式康复模式能明显减轻LDH患者的术后疼痛,减少并发症发生,促进其腰椎功能恢复,还可获得患者更满意的护理评价。
Objective To study the effect of step rehabilitation mode on lumbar function and postoperative pain in patients with lumbar disc herniation(LDH).Methods A total of 100 LDH patients who were admitted to Guangzhou Integrated Chinese and Western Medicine Hospital from February 2019 to February 2021 were selected and divided into two groups according to the random number table method,50 cases in each group.The control group was guided by routine nursing,and the observation group carried out step rehabilitation mode intervention,the satisfaction of nursing care,postoperative complications and lumbar function and pain degree in different intervention stages of the two groups were compared.Results The score of Japanese Orthopaedic Association assessment form(JOA)in observation group was higher than that of the control group at the 4 and 12th week after operation,and the score of Oswestry dysfunction index and visual analogue score(VAS)were lower than those of the control group,the differences were statistically significant(P<0.05).The total satisfaction of the patients in the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The total proportion of the patients with postoperative complications such as nerve root edema and interstitial infection in the observation group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion The step rehabilitation mode can obviously reduce postoperative pain,reduce complications and promote the recovery of lumbar function of LDH patients,and also obtain more satisfactory nursing evaluation.
