摘要
目的探讨西格列汀片联合黄葵胶囊治疗早期糖尿病肾病(DN)的临床效果及对胱抑素Ca(CysC)、同型半胱氨酸(Hcy)及尿微量白蛋白(mALB)的影响。方法选取2018年9月—2019年6月到医院门诊诊治的62例早期DN患者作为研究对象。根据随机分组法,将所有患者分为观察组和对照组,每组31例。对照组采用西格列汀片治疗,观察组采用西格列汀片联合黄葵胶囊治疗。比较两组间临床疗效、肾功能指标、血糖水平、中医症状积分、CysC、Hcy、mALB水平、炎性因子及不良反应发生率。结果观察组患者治疗总有效率(93.5%,29/31)显著高于对照组(83.9%,26/31)(P<0.05)。治疗后,观察组患者肌酐(SCr)、尿素氮(Bun)、24 h尿蛋白定量(Upro)、肾小球滤过率(GFR)、尿白蛋白排泄率(UAER)水平显著低于对照组(P<0.05)。观察组患者空腹血糖(FBG)、餐后血糖(PBG)、糖化血红蛋白(HbA1c)水平显著低于对照组(P<0.05)。观察组患者面部浮肿、尿浊、倦怠乏力、腰膝酸软症状积分均显著低于对照组(P<0.05)。观察组患者高迁移率族蛋白B1(HMGB1)、单核细胞趋化蛋白1(MCP-1)、可溶性细胞间黏附分子1(sI-CAM-1)水平显著低于对照组(P<0.05)。观察组患者HMGB1、sICAM-1、MCP-1水平显著低于对照组(P<0.05)。两组不良反应发生率差异无统计学意义(P>0.05)。结论西格列汀片联合黄葵胶囊治疗早期DN疗效确切,能有效降低血糖,改善肾功能,降低炎性因子水平。
Objective To explore the effect of Sitagliptin Tablets combined with Huangkui Capsule(黄葵胶囊)on early diabetic nephropathy(DN)and its effects on cystatin C(CysC),homocysteine(Hcy)and urine microalbumin(mALB).Methods Sixty-two early DN patients admitted to our hospital from September 2018 to June 2019 were selected as the research objects.According to the random grouping method,all patients were divided into observation group and control group,31 cases in each group.The control group was treated with Sitagliptin Tablets,and the observation group was treated with Sitagliptin Tablets combined with Huangkui Capsule.The clinical efficacy,renal function indexes,blood glucose level,TCM symptom score,levels of CysC,Hcy and mALB,inflammatory factors and incidence of adverse reactions were compared between the two groups.Results The total effective rate in the observation group(93.5%,29/31)was significantly higher than that in the control group(83.9%,26/31)(P<0.05).After treatment,the levels of Scr,Bun,Upro,GFR and UAER in the observation group were significantly lower than those in the control group(P<0.05).The levels of FBG,PBG and HbA1c in the observation group were significantly lower than those in the control group(P<0.05).The observation group's facial puffiness,urine turbidity,tiredness and fatigue,sore waist and knees scores were significantly lower than those of the control group(P<0.05).The levels of CysC,Hcy and mALB in the observation group were significantly lower than those in the control group(P<0.05).The levels of HMGB1,sICAM-1 and MCP-1 in the observation group were significantly lower than those in the control group(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Sitagliptin Tablets combined with Huangkui Capsule have a definite therapeutic effect on early DN,which can effectively reduce blood sugar,improve renal function and reduce inflammatory factor levels.
