摘要
目的研究中老年骨关节炎患者肿瘤坏死因子-α(TNF-α)、钙网织蛋白(CRT)表达,以及二者与疾病严重程度的相关性。方法选取2018年3月至2020年2月该院收治的中老年骨关节炎患者95例为病例组,进一步分为中度组30例,重度组46例,极重度组19例。另外选取因交叉韧带断裂行手术治疗的患者60例为对照组。获取所有研究对象关节滑膜组织和滑膜液,采用酶联免疫吸附试验法检测滑膜液中TNF-α、CRT表达水平,免疫组化法检测滑膜组织中TNF-α、CRT表达情况,比较不同病情患者TNF-α、CRT水平,并分析二者与疾病严重程度的相关性。结果中度组、重度组、极重度组骨关节炎患者滑膜液中TNF-α、CRT水平及滑膜组织中TNF-α、CRT的吸光度值(A值)均明显高于对照组,差异均有统计学意义(P<0.05);重度组、极重度组骨关节炎患者滑膜液中TNF-α、CRT水平及滑膜组织中TNF-α、CRT的A值均明显高于中度组,差异均有统计学意义(P<0.05);极重度组骨关节炎患者滑膜液中TNF-α、CRT水平及滑膜组织中TNF-α、CRT的A值明显高于重度组,差异有统计学意义(P<0.05);TNF-α在老年骨关节炎滑膜组织中的表达位于滑膜衬里层,且有大量炎性细胞浸润,CRT的表达主要位于血管周围和滑膜衬里层,表达量较低;关节滑膜液中TNF-α、CRT水平与骨关节炎严重程度呈正相关(r=0.667,P<0.001;r=0.704,P<0.001)。结论中老年骨关节炎患者滑膜液中TNF-α、CRT水平随着病情加重呈上升趋势,二者有望成为该病临床诊断的潜在血清学指标。
Objective To investigate the expression of tumor necrosis factor-α(TNF-α)and calreticulin(CRT)in middle aged and elderly patients with osteoarthritis and their correlations with disease severity.Methods A total of 95 middle aged and elderly patients with osteoarthritis in Sichuan Orthopedic Hospital from March 2018 to February 2020 were enrolled in case group and divided into moderate group(30 cases),severe group(46 cases)and extremely severe group(19 cases).In addition,60 healthy people in the same period were selected as control group.The expression of TNF-αand CRT in synovial fluid and synovial tissue were detected by enzyme-linked immunosorbent assay and immunohistochemical method respectively.The TNF-αand CRT expression were compared among patients with different severity degrees,and their correlations with the disease severity were also analyzed.Results The expression,absorbance(A)value of TNF-αand CRT in moderate group,severe group and extremely severe group were significantly higher than those of control group(P<0.05).The expression levels,A value of TNF-αand CRT in patients with different severity degrees were the highest in extremely severe group,followed by severe group,and which were the lowest in moderate group(P<0.05).The expression of TNF-αin synovial tissue located in the synovial lining layer,with a large number of inflammatory cells infiltrating.The expression of CRT mainly located around the blood vessels and in the synovial lining layer,and the expression level was relatively low.The expression of TNF-αand CRT in synovial fluid of joint positively correlated with the severity of the disease(r=0.667,P<0.001;r=0.704,P<0.001).Conclusion Elevated levels of TNF-αand CRT in the synovial fluid of middle aged and elderly patients with osteoarthritis positively correlate with the severity of the disease,so the two indicators are expected to become potential serological indicators for clinical diagnosis of the disease.
