摘要
目的分析老年髋部骨折患者术后谵妄发生的影响因素。方法选取2019年6月至2021年2月南通大学附属医院收治的98例老年髋部骨折患者为研究对象,根据术后意识错乱评估法的结果将入选患者分为谵妄组和非谵妄组,比较两组患者的临床特征和临床衰弱量表(clinical frailty scale,CFS)评分,采用多因素Logistic回归分析探讨老年髋部骨折患者术后谵妄发生的独立影响因素。结果98例老年髋部骨折患者术后共有28例出现谵妄,发生率为28.6%。谵妄组患者的年龄、初中及以下文化程度、脑卒中病史、术中低氧比例、术前CFS评分均显著高于非谵妄组(均P<0.05),术前血红蛋白、白蛋白水平均显著低于非谵妄组(均P<0.05)。多因素Logistic回归分析结果表明,术前CFS评分、术前白蛋白水平、术中低氧均为老年髋部骨折患者术后谵妄发生的独立影响因素(均P<0.05)。结论约1/4的老年髋部骨折患者术后出现谵妄,术前衰弱、术前白蛋白水平、术中低氧是术后谵妄发生的独立影响因素。
Objective To investigate the influencing factors of postoperative delirium in elderly patients with hip fracture.Method Ninety-eight elderly patients with hip fracture in the Affiliated Hospital of Nantong University from June 2019 to February 2021 were enrolled for the research,and they were divided into delirium group and non-delirium group according to the results of postoperative confusion assessment method.The clinical characteristics and clinical frailty scale(CFS)score were compared between the two groups.Multivariate Logistic regression analysis was performed to analyze the independent influencing factors of postoperative delirium in elderly patients with hip fracture.Result Delirium occurred in 28 patients after operation,and the incidence was 28.6%.The age,ratio of junior high school education level,stroke history,proportion of intraoperative hypoxia and preoperative CFS score in delirium group were significantly higher than those in non-delirium group(all P<0.05),and the preoperative hemoglobin and albumin levels were significantly lower than those in non-delirium group(all P<0.05).Multivariate Logistic regression analysis showed that preoperative CFS score,preoperative albumin levels and intraoperative hypoxia were the independent influencing factors of postoperative delirium in elderly patients with hip fracture(all P<0.05).Conclusion About 1/4 of the elderly patients with hip fracture suffered from postoperative delirium.Preoperative frailty,preoperative albumin levels and intraoperative hypoxia are the independent influencing factors of postoperative delirium.
