摘要
目的研究回授法健康宣教在初产妇母乳喂养中的优越性及可行性。方法选取2018年8~12月于西安交通大学第二附属医院妇产科住院分娩的足月初产妇80例为研究对象,按入院时间分为对照组(8月1日~10月15日)和观察组(10月16日~12月31日),每组40例。对照组采用产科常规护理,观察组在对照组基础上融入回授法,两组产妇均护理观察随访至产后3个月,对照分析健康宣教的效果。结果出院时观察组母乳喂养知识评分(含母乳喂养益处、哺乳体位、含接姿势、挤奶方法等几个方面)良好及优秀的比例显著高于对照组,差异有统计学意义(80.0%vs.52.5%,χ^(2)=8.87,P<0.05);婴儿出生后3个月回访两组母乳喂养情况,结果发现观察组母乳喂养率高于对照组,差异有统计学意义(100.0%vs.57.5%,χ^(2)=21.59,P<0.05);观察组母乳喂养技巧知识以及喂养效果均优于对照组,差异有统计学意义(t值分别为2.84、14.44,P<0.05)。结论在初产妇母乳喂养宣教中融入回授法有助于提高其实施率,弥补传统的床旁单向灌输式健康宣教带来的明显不足,促进初产妇掌握母乳喂养技巧,在母乳喂养健康宣教中效果显著。
Objective To investigate superiority and feasibility of health education of breast feeding in teach-back method.Methods A total of 80 full-term primiparae who delivered in Department of Gynecology and Obstetrics,The Second Affiliated Hospital of Xi′an Jiaotong University from August to December,2018 were selected as study subjects.According to admission time,they were divided into two groups:control group(admission in a period from August 1 st to October 15 th)and observation group(admission in a period from October 16 th to December 31 st),with 40 cases in each group.The primiparae in the control group received routine maternity nursing,while those primiparae in the observation group were educated in teach-back method on the basis of routine maternity nursing conducted as the control group.The primiparae in the two groups were all followed up till to 3 months after delivery.The effect of health education for the primiparae was compared between the two groups.Results The proportion of the primiparae with“good”and“excellent”scores in“breastfeeding knowledge”(breastfeeding benefits,breastfeeding position of the mother and the baby,nipple-containing posture,milking method of the mother,etc.)in the observation group was higher than that in the control group after health education in teach-back method,and the difference was statistically significant(80.0%vs.52.5%,χ^(2)=8.87,P<0.05).In follow up at 3 months after birth of the baby,it was found that the breastfeeding rate in the observation group was higher than that in the control group,the difference was statistically significant(100.0%vs.57.5%,χ^(2)=21.59,P<0.05).The knowdlege of the primiparae in feeding skills and breastfeeding effect in the observation group were better than those in the control group,and the differences were statistically significant(t=2.84 and 14.44 respectively,both P<0.05).Conclusion The application of teach-back method in breastfeeding education is helpful to improve implementation rate of breastfeeding,make up for the convent
