摘要
目的探讨肝宁片联合恩替卡韦治疗甲胎蛋白升高乙型肝炎疗效和对患者血清甲胎蛋白(AFP)及肝功能的影响。方法选取南通大学附属如皋市医院于2016年1月至2019年12月收治的60例发生AFP过度表达的乙型肝炎患者为研究对象,随机分为治疗组和对照组各30例。对照组患者采用恩替卡韦治疗,治疗组在对照组基础上联合使用肝宁片治疗,比较两组患者治疗效果,检测并比较治疗前及治疗后12、24、48周后两组患者的AFP、HBV DNA拷贝数以及肝功能水平。结果两组患者分别治疗12、24和48周,治疗组谷丙转氨酶(ALT)及AFP复常率显著高于对照组(P<0.05);且两组患者治疗后HBV DNA拷贝数及AFP表达水平相比治疗前均明显下降(P<0.05),治疗组下降幅度显著优于对照组(P<0.05)。两组患者治疗后12、24、48周后肝、脾弹性指数以及门静脉内径相比治疗前均明显下降(P<0.05),且治疗组下降幅度显著优于对照组(P<0.05)。两组患者治疗期间均未出现严重不良反应,两组患者出现的轻微恶心、失眠、头晕及腹部不适等不良反应发生率差异无统计学意义(P>0.05)。结论肝宁片联合恩替卡韦治疗AFP升高乙型肝炎的临床疗效显著,可以有效减轻肝脏炎症反应和纤维化程度,改善肝细胞增生促进肝功能恢复。
Objective To investigate the efficacy of Ganning tablets combined with entecavir in the treatment of hepatitis B with elevated alpha-fetoprotein,and the effect of alpha fetoprotein (AFP) and liver function.Methods A total of 60 hepatitis B patients with alpha-fetoprotein overexpression who were admitted to the Rugao Hospital of Nantong University from January 2016 to December 2019 were selected as the research objects,and they were randomly divided into a treatment group and a control group with 30 cases each.The control group was treated with entecavir,and the treatment group was treated with Ganning tablets on the basis of the control group.The treatment effects of the two groups were compared,and the alpha-fetoprotein levels of the two groups were detected and compared before and after 12,24,and 48 weeks after treatment.HBV DNA copy number and liver function level.Results Two groups of patients were treated for 12,24 and 48 weeks respectively.The normalization rate of alanine aminotransferase (ALT) and alpha-fetoprotein (AFP) in the treatment group was significantly higher than that in the control group (P<0.05);and the two groups After treatment,the HBV DNA copy number and AFP expression level of the patients were significantly decreased compared with before treatment (P<0.05),and the decline in the treatment group was significantly better than that of the control group (P<0.05).After 12,24 and 48 weeks of treatment,the elasticity index of liver and spleen and portal vein diameter of the two groups were significantly decreased (P<0.05),and the decrease of the treatment group was significantly better than that of the control group (P<0.05).No serious adverse reactions occurred in the two groups of patients during treatment,and there was no significant difference in the incidence of adverse reactions such as mild nausea,insomnia,dizziness,and abdominal discomfort in the two groups (P>0.05).Conclusion Ganning tablets combined with Entecavir has a significant clinical effect in the treatment of hepatitis B with e
