摘要
目的研究肾内科患者医院感染特点及其高危因素,构建危险预测模型。方法通过回顾性分析和多因素回归方法,对某医院肾内科住院患者医院感染危险因素进行调查,并构建和评价感染危险因素预测模型。结果本次调查肾内科住院患者4046例,发生医院感染患者191例,医院感染发生率为4.72%。多因素分析显示,肾内科患者医院感染的高危因素包括高龄、合并多种基础疾病、有创性操作、使用呼吸装置、联合多种抗菌药物、肾衰竭、长期卧床等。ROC曲线评价结果显示模型预测准确率高达89.40%。结论肾内科患者的医院感染危险因素复杂,可通过构建预测模型,提早采取针对性预防措施。
Objective To study the characteristics and high risk factors of nosocomial infection in nephrology patients,and to construct a model for risk prediction.Methods Through retrospective analysis and multivariate regression methods,the risk factors of nosocomial infection in inpatients in the nephrology department of a hospital were investigated,and a predictive model of infection risk factors was constructed and evaluated.Results In this investigation,there were 4046 hospitalized patients in the department of nephrology,and 191 patients with nosocomial infection.The incidence of nosocomial infection was 4.72%.Multivariate analysis showed that high-risk factors for nosocomial infections in nephrology patients included advanced age,multiple underlying diseases,invasive procedures,use of respiratory device,combination with multiple antibacterial drugs,renal failure,and long-term bed rest.ROC curve evaluation results showed that the accuracy rate of this model prediction was as high as 89.40%.Conclusion The risk factors for nosocomial infections in nephrology patients are complex,and targeted preventive measures can be taken early by constructing a predictive model.
