摘要
目的:研究小组工作介入在养老机构老年高血压患者管理中的效果。方法:选取宁波市某养老机构中年龄≥65岁的高血压患者,分为2组:对照组(常规治疗,n=38)和干预组(小组工作介入+常规治疗,n=41)。工作小组由心内科医生、护士、社会工作者、心理学家和营养师组成,干预组患者需要接受6个干预项目。干预前及干预6个月后,对2组患者高血压控制情况、自我管理行为和自我效能进行评估。结果:小组工作介入6个月后,干预组收缩压控制率(收缩压下降>10mmHg)显著高于对照组(51.22%vs.23.68%),干预组自我管理中与医生的沟通评分(2.40±0.50 vs.1.65±0.51)和自我效能评分(6.66±0.63 vs.61.17±0.82)较对照组均显著提高,差异有统计学意义(P<0.05)。结论:小组工作介入对于老年患者高血压的控制,以及自我管理和自我效能的提高,具有积极的促进作用,是一种可行和有效的管理干预方法。
[Objective]To investigated the effectiveness of group visits on hypertension control in elderly patients.[Method]Hypertensive patients aged≥65 years old,recruited from a nursing home in Ningbo,were randomly assigned to 2 groups:control group received usual care(n=38)and intervention group received GV(n=41).The GV model was comprised of doctors,nurses,social workers,psychologist and nutritionists,and the patients should participate all of the 6 courses.Hypertension control,self-management behaviors,and self-efficacy were re-evaluated at the end of the intervention.[Result]The basic characteristics did not differ significantly between control group and GV group at baseline.The results indicated that after intervention,patients in GV group had significantly better systolic blood pressure control compared with control group(51.22%vs.23.68%,P<0.05).The self-management and self-efficacy scores were also improved in GV group.[Conclusion]GV model is a feasible and effective alternative for supporting self-management and self-efficacy in patients with hypertension,which hold potential for improving hypertension control in elderly patient.
