摘要
目的:对比X线片与多排螺旋CT三维重建对踝关节骨折的诊断价值,为踝关节骨折临床诊治提供参考依据。方法:选取笔者所在医院2020年1月-2021年5月收治的踝关节骨折患者185例作为研究对象,所有患者术前均接受X线片及多排螺旋CT三维重建检查,以术中探查结果作为金标准,比较X线片与多排螺旋CT三维重建对踝关节骨折的诊断价值及下胫腓骨联合损伤发生率。结果:多排螺旋CT三维重建检查在踝关节骨折Lauge-Hansen分型诊断中的敏感度、准确度均明显高于X线片检查,诊断旋前-外旋型、旋前-外展型、旋后-外旋型、旋后-内收型的特异度明显高于X线片检查,差异均有统计学意义(P<0.05)。多排螺旋CT三维重建检查下胫腓骨联合损伤的检出率要明显高于X线片检查,差异有统计学意义(P<0.05)。结论:X线片与多排螺旋CT三维重建均是诊断踝关节骨折的有效方式,而多排螺旋CT三维重建在Lauge-Hansen分型诊断及下胫腓骨联合损伤诊断中具有更大优势,值得临床重视。
Objective:To compare the diagnostic value of X-ray and multi-slice spiral CT three-dimensional reconstruction in ankle fracture,so as to provide reference basis for clinical diagnosis and treatment of ankle fracture.Method:A total of 185 patients with ankle fracture treated in our hospital from January 2020 to May 2021 were selected as the research object.All patients underwent X-ray and multi-slice spiral CT three-dimensional reconstruction before operation.Taking the results of intraoperative exploration as the gold standard,the diagnostic value of X-ray and multi-slice spiral CT three-dimensional reconstruction in ankle fracture and the incidence of lower tibiofibular joint injury were compared.Result:The sensitivity and accuracy of multi-slice spiral CT three-dimensional reconstruction in the diagnosis of Lauge-Hansen classification of ankle fracture were significantly higher than those of X-ray examination.The specificity of pronation external rotation,pronation-abduction,supination external rotation and supination adduction were significantly higher than those of X-ray examination (P<0.05).The detection rate of lower tibiofibular syndesmosis injury by multi-slice spiral CT three-dimensional reconstruction was significantly higher than that by X-ray examination (P<0.05).Conclusion:X-ray and multi-slice spiral CT three-dimensional reconstruction are effective ways to diagnose ankle fracture,and multi-slice spiral CT three-dimensional reconstruction has greater advantages in Lauge-Hansen classification diagnosis and lower tibiofibular joint injury diagnosis,which is worthy of clinical attention.
