矽肺患者支气管肺泡灌洗液中Npt、IFN-γ的表达特征研究

摘要 目的探讨不同期别矽肺患者支气管灌肺泡洗液(bronchoalveolar lavage fluid,BALF)中新蝶呤(neopterim,Npt)与干扰素-γ(infenferon-γ,IFN-γ)的含量及临床意义。方法运用酶联免疫吸附双抗体夹心法(ELISA)对2018年6月—2020年8月淄博市职业病防治院首次行支气管肺泡灌洗术的87例不同期别矽肺患者(壹期矽肺46例、贰期27例、叁期14例)BALF中Npt和IFN-γ的含量进行检测,分析不同期别矽肺患者肺功能和细胞学计数的差异及Npt和IFN-γ水平与肺功能的相关性。结果不同期别矽肺患者的肺泡灌洗液中BALF回收量、细胞总数、巨噬细胞和淋巴细胞百分数的差异均无统计学意义(均P>0.05),中性粒细百分数在矽肺壹和贰期组均高于对照组(均P<0.05),矽肺壹、贰和叁期组BALF中Npt的水平均分别显著高于对照组[(23.69±2.79)、(29.03±3.83)、(31.52±1.94)vs(18.46±1.52)mmol/L,均P<0.01],贰期组高于壹期组、低于叁期组(均P<0.05)。BALF中IFN-γ水平矽肺壹期组低于对照组[(139.42±31.84)vs(206.37±48.25)ng/L,P<0.01],贰期和叁期组均分别高于壹期组[(191.75±24.32)、(209.30±32.22)vs(139.42±31.84)ng/L,均P<0.01]。随着尘龄的增加,BALF中Npt和IFN-γ的含量降低,二者可能具有协调作用,并呈正相关(r=0.302,P<0.05)。矽肺患者BALF中Npt水平与FEV1和FEV1/FVC均呈负相关(r=-0.310、-0.256,均P<0.05),IFN-γ水平与FEV1/FVC呈负相关(r=-0.448,P<0.05)。结论BALF中Npt和IFN-γ含量的检测可能对矽肺的早期诊断和肺功能评估具有一定的参考作用。 Objective To investigate the content and clinical significance of neopterin(Npt)and interferon-γ(IFN-γ)in bronchoalveolar lavage fluid(BALF)of patients with different stages of silicosis.Methods The Npt and IFN-γ in BALF of 87 patients with silicosis of different stages(46 patients with silicosis at the first stage,27 patients with silicosis at the second stage,and 14 patients with silicosis at the third stage),who underwent bronchoalveolar lavage for the first time in Zibo Occupational Disease Prevention and Treatment Hospital from June 2018 to August 2020,were detected by the enzyme-linked immunosorbent assay(ELISA),to analyze the differences in lung function and cell count among patients in different stages of silicosis and the correlation between the levels of Npt,IFN-γ and lung function.Results There were no statistically significant differences in the amount of BALF recovery,total cells count and the percentage of macrophages and lymphocytes in BALF among patients with different stages of silicosis(all P>0.05).The percentages of neutrophils in the first and second stage groups were higher than that in the control group(all P<0.05).The levels of Npt in BALF in the first,second and third groups were significantly higher than that in the control group,respectively[(23.69±2.79),(29.03±3.83),(31.52±1.94)vs(18.46±1.52)mmol/L,all P<0.01],while that in the second group was higher than that in the first and lower than that in the third group(all P<0.05).The level of IFN-γin BALF in the first group was lower than that in the control group[(139.42±31.84)vs(206.37±48.25)ng/L,P<0.01],while that in the second and third groups was higher than that in the first group respectively[(191.75±24.32),(209.30±32.22)vs(139.42±31.84)ng/L,all P<0.01].With the increase of dust exposure age,the levels of Npt and IFN-γ in BALF decreased,and there may be a coordination effect and a positive correlation between the two(r=0.302,P<0.05).The level of Npt in BALF in silicosis patients was negatively correlated with FEV1 and FE
