同型半胱氨酸及叶酸代谢基因MTHFR多态性与复发性流产发病风险的关联性 被引量:14

Association between homocysteine and folate metabolism gene MTHFR polymorphisms with onset risk of recurrent spontaneous abortion

摘要 目的探讨生殖道病原体感染、同型半胱氨酸(Hcy)及亚甲基四氢叶酸还原酶(MTHFR)基因多态性与复发性流产(RSA)发病风险的关联性。方法选取2019年1月-2020年7月因RSA于医院就诊的患者126例为研究组,同期就诊因男性因素致不孕,拟人工授精的女性115例为对照组,经细菌培养法检测宫颈分泌物中解脲支原体(UU),取外周血进行血清叶酸、Hcy水平测定,MTHFR C677T基因多态性检测,对比两组间各项指标。结果研究组UU感染率高于对照组(P<0.05);研究组血清叶酸水平低于对照组,而血清Hcy水平高于对照组(P<0.05);研究组C677T位点CC基因型频率低于对照组,CT基因型、TT基因型频率高于对照组(P<0.05);研究组中,CC基因型患者血清Hcy水平及UU感染率低于CT基因型、TT基因型患者,CT基因型、TT基因型患者血清Hcy水平及UU感染率差异有统计学意义(P<0.05)。结论生殖道感染、Hcy及叶酸代谢基因MTHFR(C677T)多态性与RSA发病风险有密切关联,且生殖道感染、Hcy、MTHFR C677T基因多态性间相互影响共同参与RSA发生发展。 OBJECTIVE To investigate the relationship of genital tract pathogen infection,homocysteine(Hcy)and methylene-tetrahydrofolate reductase(MTHFR)polymorphisms and the risk of recurrent spontaneous abortion(RSA).METHODS A total of 126patients treated in Ningbo First Hospital and the Affiliated Hospital of Ningbo University School of Medicine due to RSA between Jan.2019and Jul.2020were selected as the study group,and 115women who were to be artificially insemated dut to male factor infertility were selected as the control group.The Ureaplasma urealyticum(UU)in cervical secretions were detected by bacterial culture method,peripheral blood samples were collected to determine serum folic acid and Hcy levels,and MTHFR C677Tpolymorphisms.The various indicators between the two groups were compared.RESULTS The UU infection rate in the study group was significantly higher than that in the control group(P<0.05).The serum folic acid level in the study group was significantly lower than that in the control group,while the serum Hcy level was significantly higher than the control group(P<0.05).The frequency of CC genotype at C677Tlocus in the study group was significantly lower than that in the control group,while frequencies of CT genotype,TT genotype,were significantly higher than those in the control group(P<0.05).In the study group,the serum Hcy level and UU infection rate of patients with CC genotype were lower than those of CT genotype and TT genotype.The difference in serum Hcy level and UU infection rate of patients with CT genotype and TT genotype were significant(P<0.05).CONCLUSION Genital tract infection,Hcy and folate metabolism gene MTHFR(C677T)polymorphisms were closely related to the risk of RSA,and genital tract infection,Hcy and MTHFR C677Tgene polymorphisms were interacted with each other and involved in the development of RSA.
