摘要
目的研究系统免疫炎症指数(SII)与哮喘严重程度的相关性。方法采用回顾性研究方法,纳入92例处于慢性持续期的哮喘患者,将其分为轻度(27)、中度(28)、重度(37)三组,收集患者的血常规、C反应蛋白,并计算SII,检测一秒用力呼气容积(FEV_(1))、一秒用力呼气容积占预计值百分比(FEV_(1)%)、一秒用力呼气容积/用力肺活量(FEV_(1)/FVC)、呼气流量峰值(PEF)。结果SII与哮喘严重程度的相关性,且存在统计学差异(P<0.05)。SII与中性粒细胞计数呈正相关(r=0.75,P<0.05),SII与C反应蛋白呈正相关(r=0.70,P<0.05),SII与FEV_(1)%呈负相关(r=-0.46,P<0.05),SII、中性粒细胞计数、C反应蛋白对诊断重度哮喘的曲线下面积分别为0.804、0.688、0.731(均P<0.05),SII诊断重度持续期哮喘的最佳临界值为978.04,灵敏度67.6%,特异性80%。结论SII与哮喘严重程度相关,对评估重度持续期哮喘具有一定的临床价值。
Objective To study the correlation between systemic inflammatory index(SII)and asthma severity.Methods A retrospective study was conducted in 92 patients with chronic persistent asthma,who were divided into the mild group(n=27),the moderate group(n=28)and the severe group(n=37).The levels of FEV_(1),FEV_(1)%pred,FEV_(1)/FVC,PEF,C-reactive protein and blood routine were determined,and SII was calculated.Results SII increased with the progression of asthma,and there were statistically significant differences(P<0.05).SII was positively correlated with neutrophil count(r=0.75,P<0.05).SII was positively correlated with C-reactive protein(r=0.70,P<0.05),SII was negatively correlated with FEV_(1)%(r=-0.46,P<0.05).The area under the curve of C-reactive protein,SII and neutrophils for diagnosis of severe asthma was 0.804,0.688 and 0.731,respectively.(all P<0.05).The optimal cut-off value of SII for the diagnosis of severe persistent asthma was 978.04,with a sensitivity of 67.6%and specificity of 80%.Conclusion SII is related to the severity of asthma,and it has certain clinical value in the evaluation of severe persistent asthma.
