摘要
目的:评价某三甲儿童医院临床住院科室运行效率,为新时代下儿童医院优化资源配置提供决策依据。方法:整理统计医院2018-2020年26个临床住院科室运营数据,采用数据包络分析法的BCC模型分析其综合效率、纯技术效率、规模效率、相对有效性和规模报酬。结果:2018-2020年医院综合效率分别为0.977、0.972、0.890,2018-2019年医院综合效率主要受纯技术效率制约,2020年受纯技术效率、规模效率共同制约,其中外科综合效率的逐年表现优于内科。在产出不变的情况下,该院在人员经费支出、医疗业务支出和平均开放床位数上的3年平均冗余率分别为2.69%、15.14%、0.15%。结论:结合应用DRGs指标和DEA分析为医院管理者提供了科学量化的评价体系,推进医院精细化管理。新时代儿童医院应根据实际情况控制各科室规模,优化资源配置,同时加强学科建设,提高医疗技术水平,以提升医院总体运营效率;完善绩效考核方式,在维持高效运营的同时,确保医疗服务质量。
Objective: To evaluate the operational efficiency of the inpatient department of a Grade Ⅲ hospital children’s hospital, and to provide decision-making basis for optimizing resource allocation of children’s hospitals in the new era. Methods: The operation data of 26 inpatient departments in the hospital from 2018 to 2020 were collected and statistically analyzed, and the comprehensive efficiency, pure technical efficiency, scale efficiency,relative effectiveness and return to scale were analyzed using the BCC model of data envelopment analysis. Results:From 2018 to 2020, the comprehensive efficiency of the hospital was 0.977, 0.972 and 0.890, respectively. From 2018 to 2019, the hospital’s comprehensive efficiency was mainly restricted by pure technical efficiency, and in 2020, it was jointly restricted by pure technical efficiency and scale efficiency. The overall efficiency of surgery was better than that of internal medicine year by year. Under the condition of constant output, the three-year average redundancy rates in personnel expenditure, medical service expenditure and the average number of open beds are 2.69%, 15.14% and 0.15%, respectively. Conclusion: The combination of DRGs index and DEA provides a scientific and quantitative evaluation system for hospital managers, and promotes the fine management of hospitals. In the new era, children’s hospitals should control the scale of each department according to the actual situation, optimize the allocation of resources, strengthen discipline construction and improve the level of medical technology, so as to enhance the overall operation efficiency of the hospital. Improve the performance appraisal method, and ensure the quality of medical services while maintaining efficient operation.
