摘要
目的与手工法对比,评价迈瑞BC-6900全自动血细胞分析仪体液模式对胸腹腔积液细胞计数及分类的准确性。方法收集2017年7月至2018年3月某肿瘤专科医院患者胸腹腔积液标本112例。应用全自动血细胞分析仪体液模式(仪器法)与改良牛鲍计数板(手工法)进行红细胞、有核细胞计数及分类,观察两种方法的计数结果的相关性和偏差。结果无论血性或非血性胸腹腔积液标本,红细胞(RBC-BF)手工法与仪器法计数均有较强的相关性(rs分别为0.989、0.973)。与手工法比较,血性标本红细胞仪器法计数较非血性标本一致性好,偏差分别为-8.9%~27.6%、-27.6%~25.3%。有核细胞总数≤100×10^(6)/L时,有核细胞(TC-BF)、单个核细胞(MN)、多个核细胞(PMN)仪器法与手工法计数相关性(rs分别为0.704、0.792、0.761),低于计数>100×10^(6)/L区间内的相关性(rs分别为0.958、0.930、0.925);两个区间内三者仪器法与手工法计数的最大相对负偏差分别为-60%~-42.3%、-25.2%~-22.1%,最大正偏差分别为41.1%~54.5%、28.7%~29.7%。结论迈瑞BC-6900全自动血细胞分析仪体液模式红细胞计数和在有核细胞数>100×10^(6)/L时的TC-BF、MN、PMN计数与手工法比较具有良好的一致性。
Objective To evaluate the accuracy of body fluid mode of the Mindray automatic hematology analyzer for cell counting and classification in pleural and ascitic effusion cells compared to manual method with modified Neubauer hemocytometer.Methods 112 serous cavity effusion specimens were collected from patients in a Cancer Hospital from July,2017 to March,2018.Red blood cells and nucleated cells were counted and classified using automated and manual method.The correlation and deviation of the two methods were evaluated.Results There was a strong correlation between manual and instrumental counting for red blood cells(RBC-BF)in both bloody and non-bloody serous cavity effusion samples(r>0.9).The agreement between the automated and the manual method showed a high correlation,with Spearman correlation coefficients greater than 0.9 for the counting of nucleated cells(TC-BF),mononuclear cells(MN)and polymorphonuclear cells(PMN)when total number of nucleated cells>100×10^(6)/L.The correlation with the manual method was poor(r<0.8)when red blood cell(RBC)or nucleated cells≤100×10^(6)/L.The deviations to the manual method for TC-BF,MN and PMN were less in the group with nucleated cells>100×10^(6)/L.Conclusion The red blood cell and the TC-BF,MN and PMN counting by the body fluid mode of Mindray BC-6900 automatic hematology analyzer is in a good agreement with those by manual method when the nucleated cell>100×10^(6)/L.
