摘要
目的分析外源性表达潜伏膜蛋白2A(LMP2A)、Yes相关蛋白(YAP)及P27蛋白在鼻咽癌(NPC)中的表达及与临床病理特征、预后的关系。方法收集2016年1月—2018年1月我院行手术收治的NPC 81例活检术石蜡标本作为NPC组,46例鼻炎黏膜慢性炎组织标本作为对照组。收集2组患者临床资料,比较LMP2A、YAP及P27蛋白表达情况;分析LMP2A、YAP及P27蛋白与NPC患者病理参数间的相关性,采用多因素Logistic回归分析探讨影响NPC患者预后生存的因素,绘制Kaplan-Meier生存曲线研究LMP2A、YAP及P27蛋白对NPC患者预后生存的影响。结果NPC组LMP2A、YAP高表达率、P27蛋白低表达率高于对照组(P<0.01)。组织高分化、淋巴结无转移、TNM分期Ⅰ+Ⅱ期、无远处转移、非角化性未分化型NPC患者LMP2A、YAP高表达率及P27蛋白低表达率较组织中低分化、存在淋巴结转移、TNM分期Ⅲ+Ⅳ期、远处转移、非角化性分化型NPC患者低(P<0.05,P<0.01)。组织分化程度、病理分型、TNM分期、淋巴结转移及LMP2A、YAP、P27蛋白表达情况为NPC患者预后生存的独立危险因素(P<0.05,P<0.01)。LMP2A、YAP低表达组平均生存时间长于高表达组(P<0.01),P27蛋白高表达组平均生存时间长于低表达组(P<0.01)。结论LMP2A、YAP在NPC患者中表达升高,P27蛋白表达降低;LMP2A、YAP高表达及P27蛋白低表达与NPC发生、病情进展及预后有密切联系,可作为预测NPC患者预后的分子标志物和潜在治疗靶点。
Objective To analyze expressions of exogenous latent membrane protein 2A(LMP2A),Yes-associated protein(YAP)and P27 protein in patients with nasopharyngeal carcinoma(NPC)and their relationships with clinicopathological characteristics and prognoses.Methods A total of 81 biopsy paraffin specimens of NPC of who had undergone surgeries between January 2016 and January 2018 were collected as NPC group,and 46 chronic inflammatory tissue specimens of rhinitis mucosa were selected as control group.Clinical data was collected,and expressions of LMP2A,YAP and P27 proteins were compared in two groups.Correlations between LMP2A,YAP and P27 proteins with pathological parameters of NPC patients were analyzed.Multiple Logistic regression analysis was used to explore the factors affecting prognoses and survival of NPC patients,and Kaplan Meier survival curve was drawn to explore effects of LMP2A,YAP and P27 proteins on prognoses and survival of NPC patients.Results The high expression rates of LMP2A and YAP proteins and the low expression rate of P27 in NPC group were significantly higher than those in control group(P<0.01).The high expression rates of LMP2A and YAP proteins and the low expression rate of P27 protein in NPC patients with well-differentiated tissues,no lymph node metastasis,TNM stageⅠ+Ⅱ,no distant metastasis and non-keratinizing undifferentiated carcinoma were significantly lower than those with moderate-low differentiation,lymph node metastasis,TNM stageⅢ+Ⅳ,distant metastasis and non-keratinizing differentiated carcinoma(P<0.05,P<0.01).The degree of tissue differentiation,pathological classification,TNM staging,lymph node metastasis and expressions of LMP2A,YAP and P27 proteins were independent risk factors affecting prognoses and survival of NPC patients(P<0.05,P<0.01).The average survival time in low expression LMP2A and YAP group was significantly longer than that in high expression LMP2A and YAP group(P<0.01),which was significantly longer in high expression P27 group than that in low expression P27
