功能性电刺激循环运动联合低频重复经颅磁刺激对脑梗死患者上肢功能的影响 被引量:7

Effect of functional electrical stimulation cyclic exercise combined with low frequency RTMS on upper limb function of cerebral infarction patients

摘要 目的探讨功能性电刺激循环(FESC)运动联合低频重复经颅磁刺激(RTMS)对脑梗死恢复期患者上肢功能的影响。方法前瞻性纳入86例脑梗死恢复期患者,采用随机数字表法分为2组,对照组(n=43)接受低频RTMS治疗,联合组(n=43)接受FESC运动联合低频RTMS治疗。治疗2周后比较2组患者偏瘫上肢功能测试分级、上肢Fugl-Meyer评分、改良Barthel指数及安全性。结果联合组治疗前偏瘫上肢功能测试分级、上肢Fugl-Meyer评分、上肢Fugl-Meyer评分分别为(2.63±1.84)级、(29.38±5.21)分、(45.16±12.41)分,治疗后分别为(4.56±1.05)级、(40.16±5.18)分、(62.16±12.62)分;对照组治疗前偏瘫上肢功能测试分级、上肢Fugl-Meyer评分、上肢Fugl-Meyer评分分别为(2.71±1.01)级、(30.48±4.35)分、(43.71±10.79)分,治疗后分别为(3.93±1.06)级、(37.42±4.09)分、(56.06±9.28)分。与治疗前比较,治疗后2组患者偏瘫上肢功能测试分级、上肢Fugl-Meyer评分、改良Barthel指数明显升高(t=5.974、9.622、6.298、5.464、7.622、5.690,P<0.001)。与对照组比较,治疗后观察组偏瘫上肢功能测试分级、上肢Fugl-Meyer评分、改良Barthel指数明显升高(t=2.769、2.722、2.554,P=0.007、0.008、0.014)。2组患者均无明显不适症状。结论与单独低频RTMS相比,FESC运动联合低频RTMS能显著改善脑梗死恢复期患者上肢功能,提升日常生活活动能力,安全性高。 Objective To investigate the effects of functional electrical stimulation cycling(FESC)exercise combined with low frequency repetitive transcranial magnetic stimulation(RTMS)on upper limb function in patients recovering from cerebral infarction.Methods Totally 86 patients recovering from cerebral infarction were prospectively selected and divided into two groups according to the random number table method,with the control group(n=43)receiving low-frequency RTMS and the combined group(n=43)receiving FESC exercise combined with low-frequency RTMS.The hemiplegic upper limb functional test grading,upper limb Fugl-Meyer score,modified Barthel index and safety between the two groups after 2 weeks of treatment were Compared.Results The hemiplegic upper limb functional test score,upper limb Fugl-Meyer score and upper limb Fugl-Meyer score were respective(2.63±1.84),(29.38±5.21)and(45.16±12.41)before treatment and(4.56±1.05),(40.16±5.18)and(62.16±12.62)after treatment in the combination group;in the control group,the hemiplegic upper limb functional test classification,upper limb Fugl-Meyer score and upper limb Fugl-Meyer score were(2.71±1.01)grade,(30.48±4.35)points,(43.71±10.79)points before treatment and(3.93±1.06)grade,(37.42±4.09)points,(56.06±9.28)points after treatment.The hemiplegic upper limb functional test classification,upper limb Fugl-Meyer score,and modified Barthel index were significantly higher in both groups after treatment compared to pre-treatment(t=5.974,9.622,6.298,5.464,7.622,5.690,all P<0.001).Compared to the control group,the hemiplegic upper limb functional test classification,upper limb Fugl-Meyer score and modified Barthel index were significantly higher in the observation group after treatment(t=2.769,2.722,2.554,P=0.007,0.008,0.014).No significant symptoms of discomfort were observed in either group.Conclusion Compared with low-frequency RTMS alone,FESC exercise combined with low-frequency RTMS can significantly improve upper limb function and enhance activities of daily living in
