摘要
膝关节骨性关节炎(knee osteoarthritis,KOA)是一种以膝关节软骨退变和继发性骨质增生为主要特征的慢性骨关节疾病,影响患者膝关节的正常生理功能和日常活动,给患者带来极大的痛苦和经济负担。KOA可分为原发性和继发性两类。原发性的发病机制尚未完全明确,无明确的全身或局部诱因,一般认为与遗传和体质有一定关系;而继发性的发病原因较多,如创伤、炎症、先天性疾病、骨软骨发育不全;遗传性疾病,如黑尿症累及关节病变;肥胖、糖尿病等代谢性疾病等。近年来对KOA相关基因的研究发现,有大量基因在KOA中表达异常,基因的表达异常可能导致KOA的高发。本文就KOA的相关致病因素及相关基因表达研究现状进行综述。
Knee osteoarthritis(KOA)is a chronic joint disease characterized by knee cartilage wear and tear degradation and secondary bone hyperplasia. Damage and impact on the normal physiological functions and daily activities of patients’ knee joints bring great pain and economic burden to patients. KOA can be divided into primary and secondary categories. The pathogenesis of primary KOA is not completely clear,there are no clear systemic or local incentives,and it is generally considered to have a certain relationship with heredity and physique. There are many causes of secondary KOA,which can be secondary to trauma,inflammation,and congenital diseases,such as osteochondral dysplasia;genetic diseases,such as melanosis involving joint disease;obesity,diabetes and other metabolic diseases. It is the result of the interaction of multiple pathogenic factors including mechanical and biological factors. In recent years,with the study of KOA-related genes,it has been found that a large number of genes are abnormally expressed in KOA. The abnormal expression of genes may lead to a high incidence of KOA. This article reviews the related pathogenic factors and gene expression of knee osteoarthritis.
