摘要
目的:探讨高血压、糖尿病及高血压合并糖尿病患者的肾功能不全的相关危险因素。方法:来自上海市健康管理系统中的慢性病管理患者2678人,其中男性1217人,女性1461人;小于60岁94人,60~69岁1099人,70~79岁1129人,80岁及以上35人;高血压组患者的占比为60.98%,糖尿病组患者占比为19.01%,高血压合并糖尿病组患者占比为20.01%。比较高血压、糖尿病及高血压合并糖尿病3组患者的肾功能不全情况,分析合并肾功能不全的相关因素。结果:在2678名患者中发现肾功能不全的比例为12.13%。高血压、糖尿病及高血压合并糖尿病3组患者在肾小球滤过率分期、体重指数、空腹血糖、肌酐、尿酸、低密度脂蛋白胆固醇、总胆固醇、估算肾小球滤过率水平及收缩压、舒张压方面的差异均有统计学意义(P<0.05);logistic回归分析结果显示,性别、年龄、饮酒、血糖及尿酸水平是肾功能不全的主要影响因素。结论:高血压和糖尿病患者中肾功能不全的比例较高,在社区工作中需要加大对社区血糖控制不良、尿酸异常等人群的干预宣教,做到定期体检,早发现、早干预,延缓或阻断社区慢性病人群肾脏并发症的发生发展。
Objective:To investigate the relevant risk factors of renal insufficiency in patients with hypertension,diabetes and hypertension combined with diabetes.Methods:2678 chronic disease management patients from Shanghai health management system,including 1217 males and 1461 females;94 people were under the age of 60,1099 people aged 60~69,1129 people aged 70~79,and 35 people aged 80 and over;and the proportion of patients in the hypertension group was 60.98%,the proportion of patients in the diabetes group was 19.01%,the proportion of patients in the hypertension combined with diabetes group was 20.01%.The renal insufficiency of 3 groups of hypertension,diabetes and hypertension combined with diabetes were compared to analyze the related factors of renal insufficiency.Results:Among 2678 patients,the proportion of renal insufficiency was 12.13%.There were significant differences in glomerular filtration rate staging,body mass index,fasting blood glucose,creatinine,uric acid,low density lipoprotein cholesterol,total cholesterol,estimated glomerular filtration rate,systolic blood pressure and diastolic blood pressure between the 3 groups of hypertension,diabetes and hypertension combined with diabetes(P<0.05).Logistic regression analysis showed that gender,age,alcohol consumption,blood glucose and uric acid levels were the main influencing factors of renal insufficiency.Conclusion:In the patients with hypertension and diabetes the proportion of renal dysfunction is higher,in the community work,it is necessary to increase the community intervention and education for people with poor blood sugar control and abnormal uric acid,and do regular physical examination for early detection and early intervention to delay or block the occurrence and development of renal complications in community chronic patients.
