Analysis of etiologic and molecular characteristic of hand,foot,and mouth disease in Kunming City in 2019

摘要 目的了解2019年云南省昆明市手足口病的病原学及其分子特征,为手足口病防治提供一定参考依据。方法采用半巢式逆转录聚合酶链式扩增(reverse transcription-pdy merase chain reaction,RT-PCR)方法对242份临床标本进行检测,对通用引物检测结果为阳性的PCR产物进行测序和生物信息学分析以鉴定其型别及分子特征。结果从收集到粪便样品242份中检出肠道病毒阳性166份,总阳性率为68.60%;共检测到8种肠道病毒A群病毒,其中柯萨奇病毒A6型(CV-A6,30.72%)和A4型(CV-A4,15.06%)为2个主要优势病原体,并分别属于D3和F基因型;另外,男性肠道病毒阳性率多于女性(P<0.05),且以<5岁为主。结论昆明地区存在多种肠道病毒感染或流行,且CV-A6和CV-A4为HFMD的主要病原并同属于中国流行株。 Objective To understand pathogenic and molecular characteristics of hand,foot,and mouth disease(HFMD)in Kunming,Yunnan in 2019,provide a reference for the prevention and treatment of HFMD.Methods Totally 242 clinical specimens were tested tested by semi-nested RT-PCR,and the PCR products with positive results of primer detection were detected by sequencing and bioinformatics analysis to identify the types and molecular characteristics.Results Among 242 stool specimens,166 were positive for enterovirus,and the total positive rate was 68.60%.Totally 8 A serotypes of enterovirus were detected,and Coxsackie virus A6(CV-A6,30.72%)and Coxsackie virus A4(CV-A4,15.06%)were dominant pathogens,which belong to D3 and F genotype,respectively.Moreover,the positive rate of enterovirus in males was higher than that in females(P<0.05),and mainly were children<5 years old.Conclusion There are many kinds of enterovirus infection or epidemic in Kunming.CV-A6 and CV-A4 are the main pathogens of HFMD,and belong to the epidemic strains of China.
