摘要
【目的】探讨慢性心力衰竭患者(CHF)血清生长分化因子-15(GDF-15)、脂蛋白相关磷脂酶A2(Lp-PLA2)的变化与患者心功能、心室重构的相关性。【方法】选取2017年1月至2019年3月无锡市人民医院收治的140例CHF患者(观察组),并选择同期健康志愿者70例(对照组)。比较两组血清GDF-15、Lp-PLA2、氨基末端脑钠肽前体(MT-proBNP)水平及心功能指标[左室射血分数(LVEF%)、左室短轴缩短率(FS%)、左心室质量指数(LVMI)],并分析其相关性。【结果】观察组血清GDF-15、Lp-PLA2、NT-ProBNP水平显著高于对照组,差异有统计学意义(P<0.05)。观察组LVMI水平显著高于对照组,LVEF、FS水平显著低于对照组,差异有统计学意义(P<0.05)。NYHA分级Ⅱ级、Ⅲ级、Ⅳ级的CHF患者血清GDF-15、Lp-PLA2水平呈逐渐升高的趋势,组间比较差异均有统计学意义(均F<0.05)。CHF血清GHF-15、Lp-PLA2水平与LVEF、FS均呈显著负相关性(P<0.05),与LVMI及血清NT-proBNP水平均呈显著正相关性(P<0.05)。【结论】CHF患者血清GDF-15、Lp-PLA2水平升高显著,并且与患者心功能及心室重构关系密切。
【Objective】To investigate the relationship between the changes of serum growth differentiation factor-15(GDF-15)and lipoprotein associated phospholipase A2(Lp-PLA2)and cardiac function and ventricular remodeling in patients with chronic heart failure(CHF).【Methods】A total of 140 patients with CHF Treated in Wuxi Peoples Hospital from January 2017 to March 2019(observation group)and 70 healthy volunteers in the same period(control group).The levels of serum GDF-15,Lp-PLA2,N-terminal pro brain natriuretic peptide(NT-proBNP)and cardiac function indexes[left ventricular ejection fraction(LVEF%),left ventricular short axis shortening(FS%)and left ventricular mass index(LVMI)]were compared between the two groups,and their correlation was analyzed.【Results】The levels of serum GDF-15,Lp-PLA2 and NT-proBNP in the observation group were significantly higher than those in the control group(P<0.05).The level of LVMI in the observation group was significantly higher than that in the control group,and the levels of LVEF and FS were significantly lower than that in the control group(P<0.05).The levels of serum GDF-15 and Lp-PLA2 in CHF patients with NYHA grade Ⅱ,Ⅲand Ⅳ increased gradually»and there were significant differences between the groups(mean F<0.05).The levels of serum GDF-15 and Lp-PLA2 in C'HF were significantly negatively correlated with LVEF and FS(P<0.05),and positively correlated with LVMI and serum NT-proBNP(P〈0.05).【Conclusion】The levels of serum GDF-15 and Lp-PLA2 in patients with CHF are significantly increased^which are closely related to cardiac function and ventricular remodeling.
