摘要
目的:探究治疗脾肾阳虚型溃疡性结肠炎的中药复方中,药物使用和组合的规律,并创新药物组合,为临床使用中药治疗提供参考依据。方法:于中国生物医学文献数据库、中国知网数据库、万方数据库、维普数据库进行文献检索,获取1990年1月-2018年11月期间与中药复方口服治疗溃疡性结肠炎相关的文献,筛选脾肾阳虚相关证型,最终纳入文献73篇,采用资料分析法处理后,通过中医传承辅助平台系统,探讨组方用药规律,发掘新方。结果:纳入的73篇文献涉及方剂共54种,包含中药共102味,其中药物四气温、平性占比最多,五味甘、辛、苦最多,多归于脾、胃、肾经。有18味药的频次在10次以上,其中补骨脂、白术、肉豆蔻、甘草、吴茱萸用药频率居于前五位。支持度≥21,置信度≥0.9的药物组合21个,包含核心中药7味。基于复杂系统熵聚类的药物组合分析,获得3味药核心组合11个,4味药核心组合2个,演化出3个新方组合:(1)五味子-吴茱萸-陈皮-马齿苋;(2)砂仁-桔梗-白头翁-白扁豆;(3)党参-黄芪-人参-赤石脂-粳米。结论:补气药、温里药、敛肺涩肠药在组方中使用较多,温肾健脾、涩肠止泻是治疗脾肾阳虚型溃疡性结肠炎的主要治法,而四神丸可作为治疗的基础方。
Objective:The medication rules of treating the spleen-kidney Yang(阳)deficiency type ulcerative colitis in TCM were explored,and medicine combinations were innovated,to provide more reference for treating the disease with TCM medicine.Methods:The related literatures of treating spleen-kidney Yang deficiency type of UC with oral TCM compound from January 1990 to November 2017 in CBM,CNKI,WANFANG data,and VIP database were retrieved.73 articles were included eventually.After the data processing was adopted,the TCM inheritance support system was applied to explore the prescription rules,and discover new prescriptions.Results:Among the 73 included literatures,54 kinds of prescriptions were involved,including 102 kinds of TCM medicines.Among them,medicines with mild and moderate properties accounted for the largest proportion,and sweet,pungent,and bitter of the five flavors accounted for the largest proportion,and mostly attributed to the spleen,stomach and kidney meridian.The frequency of 18 medicines was more than 10 times.Among them,Buguzhi(Psoralea corylifolia L.),Baizhu(Rhizoma Atractylodis),Roudoukou(Myristicae Semen),Gancao(Glycyrrhizae Radix et Rhizoma),and Wuzhuyu(Evodiae Fructus)were among the top five.There were 21 medicine combinations with“support≥21”and“confidence≥0.9”,including 7 core TCM medicines.Based on the medicine combinations analysis through complex system entropy clustering,11 core combinations of 3 medicines and 2 core combinations of 4 medicines were obtained,and 3 new prescription combinations were evolved.Conclusion:Medicines for invigorating Qi(气),warming purtenance,and astringing lung and intestines are used more in the prescriptions.Warming kidney and invigorating spleen,astringing intestines and relieving diarrhea are the main therapies on the spleen-kidney Yang deficiency UC,and Sishen Wan(四神丸)can be applied as the basic prescription.
