缺血性脑血管病患者脑血管支架植入手术治疗的有效性及对患者并发症发生的影响 被引量:2

Effectiveness of cerebrovascular stent implantation in the patients with ischemic cerebrovascular disease and its effect on the occurrence of complications

摘要 目的 探讨缺血性脑血管病患者脑血管支架植入手术治疗的有效性及对患者并发症发生的影响。方法选取2017年7月至2020年1月佳木斯市中心医院收治的98例缺血性脑血管病患者,随机分为两组,对照组进行常规药物治疗,研究组进行支架植入手术治疗。比较两组患者治疗效果、并发症、病变血流速度、血管狭窄程度及血管内皮功能。结果 与对照组并发症发生率26.5%相比,研究组并发症发生率4.1%更低,与对照组治疗总有效率85.7%相比,研究组治疗总有效率98.0%更高(P<0.05);与对照组椎动脉、颈总动脉、基底动脉、颈内动脉血流速度[(154.1±6.4)cm/s、(145.9±5.2)cm/s、(152.4±7.2)cm/s、(167.3±5.8)cm/s]比较,研究组椎动脉、颈总动脉、基底动脉、颈内动脉血流速度[(104.8±5.2)cm/s、(102.7±6.3)cm/s、(117.2±5.4)cm/s、(128.6±4.4)cm/s]更低(P<0.05);与对照组血管狭窄程度、一氧化氮(NO)、血管内皮生长因子(VEGF)、内皮素-1(ET-1)水平[(25.1±1.7)、(36.4±5.2)nmol/mL、(441.6±49.6)pg/mL、(503.6±52.1)pg/mL]比较,研究组血管狭窄程度、NO、VEGF水平[(13.7±1.3)、(31.2±4.1)nmol/mL、(413.6±50.5)pg/mL]更低,ET-1水平(546.2±54.9)pg/mL更高(P<0.05)。结论 对缺血性脑血管病患者进行脑血管支架植入手术治疗,可降低病变血流速度和血管狭窄程度,维持正常血管内皮功能,且治疗有效率高,并发症发生率低。 Objective To investigate the effectiveness of cerebrovascular stent implantation in patients with ischemic cerebrovascular disease and its effects on the occurrence of complications.Methods A total of 98 patients with ischemic cerebrovascular disease who were admitted to Jiamusi Central Hospital from July 2017 to January 2020 were selected and randomly divided into two groups.The control group was given conventional medication,and the study group was given stent implantation.The therapeutic effect,complications,blood flow velocity of lesion,vascular stenosis degree and vascular endothelial function were compared between the two groups.Results Compared with the complication rate of 26.5%in the control group,the complication rate was 4.1%in the study group,much lower than that in the control group.Compared with the total treatment effective rate of 85.7%in the control group,the total treatment effective rate in the study group was higher,which was 98.0%(P<0.05);compared with the blood flow velocity of the vertebral artery,common carotid artery,basilar artery and internal carotid artery in the control group[(154.1±6.4)cm/s,(145.9±5.2)cm/s,(152.4±7.2)cm/s,(167.3±5.8)cm/s],the blood flow velocity of the vertebral artery,common carotid artery,basilar artery and internal carotid artery was[(104.8±5.2)cm/s,(102.7±6.3)cm/s,(117.2±5.4)cm/s,(128.6±4.4)cm/s]was lower in the study group(P<0.05);compared with the vascular stenosis degree,nitric oxide(NO),vascular endothelial growth factor(VEGF),and endothelin-1(ET-1)levels[(25.1±1.7),(36.4±5.2)nmol/mL,(441.6±49.6)pg/mL,(503.6±52.1)pg/mL]in the control group,the degree of vascular stenosis,NO and VEGF levels[(13.7±1.3),(31.2±4.1)nmol/mL,(413.6±50.5)pg/mL]were lower,and the ET-1 level(546.2±54.9)pg/mL was higher in the study group(P<0.05).Conclusion Cerebrovascular stent implantation for the patients with ischemic cerebrovascular disease can reduce the blood flow velocity and the degree of vascular stenosis of the disease,maintain normal vascular endothelial func
