摘要
目的探讨支架管留置不同时间在经尿道输尿管镜取石术(URL)治疗泌尿系结石中的应用效果。方法2017年5月至2020年5月在我院治疗的148例泌尿系结石患者,根据URL术后支架管留置时间不同,分为对照组77例(术后留置4周)和观察组71例(术后留置2周)。比较两组结石清除率、术后并发症发生率、生活质量及肾功能指标差异。结果两组净石率比较差异无统计学意义(P>0.05);观察组下尿路感染率、肉眼血尿率、腰腹痛率均低于对照组(P<0.05);两组术后SF-36量表各项评分均升高,且观察组各项评分高于对照组(P<0.05);两组术后SCr、BUN、UA水平均较术前下降(P<0.05),但两组间上述指标比较,差异无统计学意义(P>0.05)。结论泌尿系结石患者URL术后输尿管支架管留置时间为2周的安全性较高,可降低下尿路感染、肉眼血尿等并发症发生率,提高患者的生活质量。
Objective To explore the application effect of stent indwelling for different time in the ureteroscopic lithotripsy(URL)of patients with urinary calculi.Methods One hundred and forty-eight patients with urinary calculi treated in our hospital from May 2017 to May 2020 were selected.According to the different stent indwelling time after URL,the patients were divided into a control group with 77 cases(indwelling for 4 weeks after surgery)and an observation group with 71 cases(indwelling for 2 weeks after surgery).The differences in stone clearance rate,incidence of postoperative complications,quality of life,and renal function indexes were compared between the two groups.Results There was no statistically significant difference in the stone clearance rate between the two groups(P>0.05).The rates of lower urinary tract infection,gross hematuria,and low back pain in the observation group were lower than those in the control group(P<0.05).The scores of each item on the SF-36 scale were increased in both groups after surgery,and the increase in the observation group was greater than that of the control group(P<0.05).Compared with those before operation,the levels of creatinine(SCr),urine nitrogen(BUN),and uric acid(UA)were decreased in both groups after operation(P<0.05).However,the levels of the above indicators were similar between the two groups after surgery(P>0.05).Conclusion In patients with urinary calculi,the indwelling time of the ureteral stent for 2 weeks after URL surgery is safe.This can reduce the incidence of complications such as lower urinary tract infection and gross hematuria,and improve the quality of life of the patients.
