摘要
姜树民教授认为,慢性萎缩性胃炎(chronic Atrophic gastritis, CAG)以脾胃气虚、胃阴虚为本,将CAG分为脾胃湿热型、肝胃不和型、脾胃虚弱型、胃络瘀血型。姜教授认为,CAG疾病有内痈之特征,应运用中医"治未病"思想,治疗上"以痈论治",结合"久病入络"思想,以虚-瘀-毒为理论轴,处方时搭配清热解毒之品,如连翘、苦参、蒲公英,并创立清中消痈汤;治疗CAG时应加用疏肝解郁之品,同时给予患者相关心理健康教育,善用胃镜诊察、血压监测、血糖监测、血脂监测等,尽早发现及早干预。病愈之后,姜教授认为,患者多有气血津液亏虚、脏腑不平、正气未安或余邪未净的病理特点,故用药常以护其胃气为主,兼以循其病情润其津液,恢复气血精神、脏腑功能。此外,还应考虑气候时节,治疗饮食难消发病于春季者,或用青皮、陈皮;夏季发病者加黄柏;病于秋燥之时者,气常涩滞,加槟榔、白豆蔻、砂仁;病于严寒冬季者加吴茱萸。
Professor Jiang Shu-min believes that chronic atrophic gastritis(CAG)is due to spleen-stomach qi deficiency and stomach yin deficiency as the root causes.He divides CAG into spleen and stomach dampness-heat type,liver and stomach disharmony type,spleen and stomach deficiency type and stomach collateral blood stasis type.Professor Jiang believes that CAG disease has the characteristics of internal carbuncle,so he uses the idea of"treating disease before its onset"in TCM,"treats it from carbuncle","long-term disease into collaterals",regards deficiency-blood stasis-toxin as the theoretical axis,and adds heat-clearing and detoxification products,such as Lianqiao(Fructus Forsythiae),Kushen(Radix Sophorae Flavescentis),and Pugongying(Herba Taraxaci),and has created Decoction for Clearing Middle Energizer and Removing Carbuncle.Professor Jiang believes that in the treatment of CAG,it is necessary to add the products of soothing the liver and relieving depression,give patients relevant mental health education,make good use of gastroscopy,blood pressure monitoring,blood glucose monitoring,blood lipid monitoring,etc.,find and intervene as soon as possible.After recovering from the disease,most patients have the pathological characteristics of deficiency of qi,blood and body fluid,uneven zang-fu organs,restless healthy qi or unclean residual evils.Therefore,drugs are often used to protect stomach qi,moisten body fluid,restore qi,blood and essence,and the functions of zang-fu organs.In addition,he believes that it is also necessary to consider the climate season.For those with CAG combined with indigestion in spring,Qingpi(Pericarpium Citri Reticulatae Viride)or Chenpi(Pericarpium Citri Reticulatae)can be used.For those with CAG in summer,Huangbai(Cortex Phellodendri Chinensis)is added.For those with CAG in dry autumn,combined with astringent and stagnant qi,Binglang(Semen Arecae),Baidoukou(Fructus Amomi Kravanh)and Sharen(Fructus Amomi)can be added.For those with CAG in severe cold winter,Wuzhuyu(Fructus Evodiae)can be add
